People finding it hard to manage on their own, and

their families, need to know where they can go for advice

and help on the many adult social care services available

locally. This Directory is an important source of that

information.

There are many appropriate services provided by

different organisations, including councils, voluntary

organisations, private care agencies and the NHS. In this

Directory, which covers a large area, three councils are

responsible for community care services:

• Bournemouth Borough Council (Adult Social Care),

• Borough of Poole (Adult Social Care), and

• Dorset County Council (Adult and Community Services).

Throughout the text, for ease of use, the services are

described as coming from ‘Adult Social Care’.

Where do I start?

Help and advice

Welcome

Adult Social Care assesses need and provides information,

advice and services to the people of Bournemouth, Poole

and Dorset, 24 hours a day, 365 days a year. Adult Social

Care supports all adults who have physical disabilities,

learning disabilities, mental health issues, a sensory

loss or general frailty. The aim is to actively promote

independence and choice and help people to be self-

reliant and live in their own homes for as long as possible.

When this is no longer possible they help people to

choose good quality supported housing or care homes.

If you need any form of advice about help to remain

independent, or about your care options, you should

phone your relevant Adult Social Care department to

request an assessment. Contact details can be found

below.

Anyone can ask for an assessment, regardless of their

financial situation. The assessment and advice is free. You

may be asked to make a financial contribution towards

services arranged for you, which could be the full cost of

the service for people who can afford to pay.

Adult Social Care will always provide support and advice,

even if they cannot pay for the help you need. They will do

their best to help get you back to living independently.

One in 10 people in the UK care for friends or family

members. Adult Social Care supports unpaid carers in

various ways, for example by providing training, links to

support groups and assisting carers to get a break from

their caring responsibilities.

Before they can arrange care services for you or provide

a Personal Budget for your social care, Adult Social Care

must assess your needs. Social workers, housing staff and

health professionals may work with you and your carer to

identify what your needs are and how they might best be

met.

The assessment is based on your circumstances and will

be different for each person. A social worker/care manager

will look at ways to best meet your agreed outcomes and

goals.

The best way of meeting your goals will then be

discussed with you, sometimes with an occupational

therapist’s help. If you already have a relative or friend who

is your carer, they can also be assessed if they wish.

Once your needs have been assessed and your desired

outcomes established, Adult Social Care will decide if you

are eligible for their help.

A financial assessment will also take place, to determine

how much you can afford to contribute towards the cost

of your care.

Am I eligible for an assessment?

To be eligible for an assessment, you may have one or

more of the following:

• sight or hearing difficulties;

• mental health needs;

• frailty because of old age;

• on-going care needs when you leave hospital;

• learning disabilities;

• permanent, substantial physical disabilities;

• a long term illness; or

• problems related to substance misuse.

Alternatively, you may provide regular unpaid care

for someone and subsequently be eligible for a carer’s

assessment.

Who can refer you for an assessment?

You can be referred by:

• yourself or a relative, friend or neighbour;

• a social worker or health professional such as a doctor or

health visitor; or

• any other agency.

If you wish to request an assessment or need more

information, telephone your local Adult Social Care office

(contact details are on page 4). They will arrange for an

interpreter if you do not speak English or if you use sign

language.

What does the process involve?

The assessment of your needs will involve your social

worker talking with you, your carer (if you wish) and other

people who know you well, about different aspects of

your life. These discussions may include your physical

health, your emotional health, how you manage to look

after yourself, what outcomes you are looking for from

the care and support provided and your current family

and support networks. Methods of best meeting your

needs and achieving your outcomes will be discussed and

agreed with you.

Do I have a say in what services I get?

After your needs have been assessed, Adult Social Care will

advise you about services you may be eligible for and your

Personal Budget (explained on page 8) to help you meet

your care needs. They will discuss with you:

• the type of help you think you need to meet your

outcomes;

• when you think it is needed;

• who will be providing the assistance;

• how long they will be with you;

• how it will be paid for; and

• your religious and cultural needs.

Support plans are reviewed regularly and will include all

references to any Personal Budget you may be eligible for,

to help pay for your care needs.

Your assessment


If you haven't already heard of the new Care Act, you may

well do so soon. It's a new piece of law which will come

into effect during April 2015, making positive changes to

the way care and support is provided for adults and carers.

Many of the things the Care Act now makes statutory

for councils are things we are already doing or working

towards such as having a Safeguarding Board and

outcome-based assessments. It also introduces new duties

to all councils, designed to ensure that wellbeing, dignity

and choice are at the heart of health and social care.

The changes include:

Carers: For the first time, carers will be recognised in law in

the same way as those they care for, and more will able to

have an assessment of their own support needs.

People who pay for their own care and support: Will be

entitled to an assessment under the law.

Safeguarding: Anyone at risk of abuse will be even better

protected as the Act puts Adult Safeguarding Boards on a

statutory footing.

Care costs: The Care Act also takes into account the findings

of the Dilnot Report into the funding of care and support,

introducing a number of significant changes to how care

is charged for, who has to contribute, and a cap on how

much people will have to pay towards their care. This

comes into effect from April 2016.

The reforms also include council duties on prevention

and wellbeing, information and advice, the national

minimum threshold for eligibility; Personal Budgets,

care and support plans and universal deferred payment

agreements.

The Care Act will mean that a wider range of good

quality services give you more control and help you to

make more personalised choices over your care, resulting

in even better care that works for you.

It is designed to ensure that people receive services that

prevent their care needs from becoming more serious, or

delay the impact of their needs; can get the information

and advice they need to make good decisions about care

and support; have a range of high-quality care providers to

choose from.

The new Care Act and what it means for you

National developments in care and support

A Personal Budget is the money a council allocates to

an individual to meet their care and support needs. If

you have been assessed as eligible to receive support

from Adult Social Care, you will be advised of what your

Personal Budget amount is.

Direct Payments are one of the ways that you can

choose to receive your Personal Budget. This means

having money paid into a specific Direct Payments bank

account (that you will need to set up) or into a holding

account arranged by Adult Social Care. You can then use

this money to purchase your own care, rather than the

council arranging it for you.

Personal Budgets and Direct Payments

Direct Payments gives you choice and control over your

care and you can use your Personal Budget to buy services

directly from Adult Social Care or other sources; you

cannot use it to buy healthcare or long term residential

care.

Some people use their money to buy care or support;

help with everyday tasks such as washing or getting

dressed, from agencies or organisations, or employ a

personal assistant.

The outcomes of your assessment may have identified

other areas of life; for example leisure and social activities

and in this case you may wish to purchase day care or

recreational pursuits.

Personal health budgets

These were introduced nationally in 2009 and have

continued to grow – 636,000 people already have social


care budgets and the NHS is learning from and building

on this experience.

The budgets are already available for people who are

eligible for NHS Continuing Healthcare. A personal health

budget is an amount of money to support a person's

identified health and well-being needs, planned and

agreed between the person and their local NHS team. The

aim is to enable people with long term conditions and

disabilities to have greater choice, flexibility and control

over the healthcare and support they receive.

Patients have had a 'right to ask' for a personal health

budget since April 2014 and this became a 'right to have'

from October 2014.

NHS care will continue to be free for patients at the

point of use.

Personal Budgets and Direct Payments continued

Adult Social Care and the NHS work very closely together to

ensure that hospital patients are discharged safely and are

able to return to home life as quickly as possible. Social care

staff are available to help with information and advice when

you are ready to leave hospital.

No matter which hospital you are in, you can request an

assessment of your needs to help you stay independent and

confident when you return home. The assessment involves

asking you about your needs and what is required to help

you live safely and independently at home. There will also

be an explanation of any charges that may be made.

A relative, carer, or a member of the hospital staff may

also approach the hospital social care team and ask them to

carry out an assessment, but this will not go ahead unless

you agree.

If you already have a social care service in place Adult

Social Care will make an assessment of any other needs you

may have after your stay in hospital.

Carers should be offered an assessment and may also

need advice and support. The hospital social care team is

available to discuss any issues with them and also give them

details of carers’ support groups.

Adult Social Care can also introduce you to local voluntary

organisations who can help with a variety of services such as

laundry and domestic cleaning, or sitting services.

Borough of Poole also works with people who are being

discharged from hospital who are not eligible for financial

assistance from Adult Social Care services, sometimes

known as self-funding clients.

A social worker will visit these clients and undertake a

full assessment of the person’s needs. The social worker

will help people with information, advice and support

about appropriate community services which can include

information about care homes, home care, benefits

information and day opportunities. They will also signpost

people to other services that may be able to help.

Dorset residents being discharged after a stroke from

Dorset County Hospital (Dorchester), Royal Bournemouth,

Christchurch or Poole hospitals are supported through

a Stroke Coordination service. This provides advice and

information, support and signposting to help people to

become as independent as possible.

Poole residents being discharged from Poole Hospital

following a stroke are also supported by a Stroke

Coordination service. This service provides information,

support and signposting to ensure that people have the

information they need to make choices which enable them

to live as independently as possible after a stroke. The

Stroke Coordination service can also refer clients to the care

management team and psychological support services

if additional care support is required, based on identified

need.

At Bournemouth Hospital, Bournemouth Churches

Housing Association’s Leaving Hospital Support Service

makes your return as simple and stress-free as possible.

It’s a free service funded by the Royal Bournemouth and

Christchurch Hospitals NHS Foundation Trust, Bournemouth

Borough Council, Borough of Poole, Dorset County Council

and Hampshire County Council. For more information

Leaving hospital and staying independent


NHS continuing healthcare is provided solely by the NHS if

you have extensive healthcare needs. This is free of charge,

wherever it is delivered and can be provided in any setting.

In your own home the NHS funds all care required to meet

your assessed needs. In a care home, the NHS makes a

contract with the home and pays the full fees for your

accommodation as well as all your care.

You can ask for an assessment by health and social care

professionals who will visit you to identify and prioritise

your needs. The assessment will be used to decide what

services will be provided and where, and who funds them.

This is explained further on page 7.

If your needs change, regular reviews will be carried

out and your care and/or funding arrangements will

change to meet them. Contact the Dorset Clinical

Commissioning Group (CCG) for more information (see

page 51 for contact details of the relevant office for

your region).

If you’ve had an illness, lost a partner, just come home

from hospital, or are becoming frail, you might find it

difficult looking after yourself. You may not feel strong

enough to cope, have lost confidence or simply not be

used to coping on your own. We find that many people

in this position just need a bit of help to get back on their

feet. This is called ‘reablement’.

Reablement is a short period of intensive support in

your home and there is no charge for this service. Adult

Social Care will work with you to help you regain old skills,

or develop new ones. In most cases this is all you will need

to continue to live independently in your own home.

Please note that this service is available to all adults

in Dorset and Poole, and adults aged over 65 in

Bournemouth.

If Adult Social Care thinks reablement can help, staff

will visit you at home to talk to you about it and will find

out what you want to be able to do and agree when help

is needed. Adult Social Care will then arrange support in

your own home to help you regain confidence and ability

in carrying out daily living tasks, such as:

• dressing and getting into and out of bed;

• washing and shaving;

• managing the toilet; and

• eating and drinking.

The amount of help needed varies with each person

and their circumstances. You might need only a few

visits a week or several visits a day. Over a period of

up to several weeks, the amount of help you need

should reduce quickly, as your confidence and ability

improves.

At the end of the period of reablement, most people

can live independently at home. However, if you are not

fully independent, Adult Social Care will tell you how much

more help they think you will need, how much this might

cost and whether Adult Social Care will pay something

towards the cost.

Occupational therapy

Reablement and rehabilitation

Occupational therapists can help you lead a more

active and independent life using a variety of methods,

including the use of adaptive equipment. They can assess

your home for hazards and point out things that might

contribute to falls. They also advise carers on techniques

for moving and handling the person they care for.

You may be entitled to help with equipment for daily

living or adaptations to your home if you:

• are elderly;

• have a disability;

• have a long-term illness which causes you to have

difficulties coping with daily living tasks; or

• are assessed as having eligible needs.

All provision and recommendations are subject to

assessment, explained on page 7.

Visiting you at home, an occupational therapist will

be able to suggest adaptations that will improve your

mobility. Perhaps handrails would be useful? Would a ramp

up to your front door help if you are in a wheelchair?

Major alterations such as fitting a level-access shower

or widening doorways can be paid for with a Disabled

Facilities Grant (DFG).

An occupational therapist will talk you through any

new equipment you may be given, as well as help you


apply for funding for major adaptations, if you need them.

An assessment of your needs will determine whether an

occupational therapist will be the best person to help you.

As of 1st April 2015 you can access a wide range of aids to

daily living to meet your assessed needs through the

pan-Dorset Integrated Community Equipment Service.

your assessed needs through the Equip for Living Service.

Sight and hearing services enable clients to improve their

independence and safety at home by providing access to

a wide range of sensory equipment.

Working closely with clients to manage risks to health

and safety in the home environment, new ways can be

found to enable you to undertake everyday activities

confidently and to complete them independently.

The team can provide:

• a wide range of equipment to support individuals and

offer reassurance to families and carers;

• specialist equipment to help people with a hearing loss;

• mobility training and daily living skills training for people

who are blind or partially sighted;

• registration and advice and guidance for people who are

diagnosed with a visual impairment;

• loop systems to support hearing;

• magnifiers to support reading.

There is a wide range of equipment available that can

easily be used in the home to make life easier and help

you remain independent. These things could be a stair

rail, raised toilet seating, equipment to help in the bath

or shower, or items to help you prepare and eat food and

drink. They allow you to continue:

• moving around your home;

• getting in and out of a chair or bed;

• getting in and out of the bath or shower;

• preparing food and drink; or

• reaching the front door.

How to get equipment

Equipment for short-term loan is available through the

British Red Cross Society loans service and is ideal if you

need some equipment temporarily; for example, while a

disabled relative is on holiday with you or to try out before

you buy. You can phone them on 01823 273746.

You can buy some equipment that might help you from

local shops that specialise in disability equipment. There

are also some local voluntary organisations and private

providers that sell equipment. We can tell you more about

this if you contact us.

Occupational therapy continued


Telecare and assistive technology equipment

There are many other products available to support more

vulnerable people to live as independently as possible in

the community for longer. The aim is to give people more

choice and control over how they live their lives and the

risks they are able to take. These products are often known

as ‘telecare.’

Telecare can prompt, remind and inform people and

their carers about things in daily life which need to be

done, such as taking their medication on time. They can

alert a family member or carer that a person has got out

of bed at night and needs assistance. Or, they may alert

a monitoring centre that something has happened and

appropriate action should be taken.

Assistive technology and community alarms

Assistive technology or telecare services help vulnerable

people, particularly older people, to live in their own

homes with greater safety and independence.

They use a range of technologies that can raise alarms

to get help for someone 24 hours a day and so provide

help, security and peace of mind. Some technology can

be supplied that will promote safety and independence

without necessarily having to link to a telecare service, thus

endorsing the principles of reablement. Some equipment

For some of the services you do not even have to press a

button to call for help as they work on sensors that detect

movements like falls, gas or carbon monoxide escapes, or

floods.

There are different types of assistive technology

and telecare services depending on your level of need.

Equipment can be provided if you have been assessed as

eligible for help by Adult Social Care, or equipment can be

purchased privately.

Dorset

Those assessed as eligible for telecare equipment in Dorset

will be provided with the equipment free of charge.

Equipment provided can range from a Lifeline unit and

pendant only, through to a Lifeline unit and a whole range

of additional sensors. An individual’s ability to pay the

monitoring charge will be based on a financial assessment.

Those assessed as eligible for assistive technology

the responsibility of the client to pay for their subscription

Poole Lifeline provides a compact emergency alarm

system for all people at risk living in their own homes.

The service is backed up by a local control centre

equipped with call-handling computers, which provide

two way speech communication between callers and

Bournemouth

Bournemouth’s Careline is a 24-hour monitoring and

referral service for residents who need help in an

emergency. They are accredited by the Telecare Services

Association (TSA), the regulatory body for the UK’s telecare

If you look after, help or support a person who is ill, frail

or disabled who cannot manage without support and do

not receive pay to do this then you are a carer. The person

might be a child with a disability, an adult member of your

family, or a friend. You do not need to be living with the

person. You do not have to be giving physical care.

In June 2014 a Carers Centre for carers of people living in

Bournemouth or Poole opened on Alumhurst Road in

Westbourne BH4 8ER. There are drop-in sessions at the

centre on Tuesday afternoons from 2.00pm to 4.00pm and

Friday mornings from 10.00am to 12.00am where carers

can meet for a chat and receive specialist information and

advice about local support. The Carers Information Service

keeps families and friends informed of helpful news and

events. Regular events enable carers to meet up and enjoy

an activity, some of these with the person they support.

Carers receive a free welcome pack when they join, copies

of the local Caring Matters magazine, regular newsletters

and a friendly team to call when they need help.

There are different ways carers can access a break.

Throughout the year there are beach huts available and a

luxury holiday lodge that carers can apply to stay in.

There is a short break sitting service in both Poole and

Bournemouth that runs in a similar way to the service in

Dorset and is available to unpaid carers of cared for people

who are over 18. It is a flexible service that provides a care

worker from a local approved agency to stay with the

cared for person. The service in both areas is subsidised by

the local authorities, but they are all run slightly differently.

Supporting unpaid carers

Unpaid carers sometimes need a break from their caring

responsibilities and this can be provided in several ways.

The person you care for may wish to go into a care home

for a week or two, or you may want to arrange home-

based respite care where another carer moves into your

home, or family-based respite care where they move

into another family’s home for a period. The best option

for you will depend on your needs, the urgency of the

situation and personal choice and cost.

‘Home from Home’ day respite

‘Home from Home’ offers a day time/respite service in the

host’s own home. Each host undertakes training, has an

enhanced Disclosure and Barring Service police check and

their home has been inspected to make sure it reaches

appropriate health and safety standards. The service is

managed by East Boro Housing Trust.

Carers in Crisis

As well as needing a break, carers worry about what would

happen to the person they support if the carer is unwell,

or has an accident. Carers in Crisis is an emergency back-

up scheme, available to all carers of adults over 18, that

has been set up to give carers peace of mind by alerting

people to keep the person being cared for safe.

Dorset County Council carers’ caseworkers provide

assistance to family carers to support them in their caring

role. Carers may require help in finding out information on

how to find local support groups and carers’ caseworkers

can assist them with this. Carers’ caseworkers may be able

to support family carers to access local services which may

give them a much needed break. The carers’ caseworker

will carry out an assessment with carers who provide a

substantial level of care. This may lead to a support plan for

the carer.

Staying safe from falls

Improving safety in the home can help prevent elderly

people from having falls and injuring themselves, whilst

allowing them to continue to live independently.

Falls, slips and trips can lead to broken hips or wrists

and other injuries, and are a leading cause of disability

among the older population. Falls in the home can result

in ill health, lengthy hospital stays, residential care, loss of

independence, and greatly affect quality of life. Making

elderly people’s homes safer can help prevent them falling

and seriously injuring themselves.

Various aspects of the home environment can be

improved to reduce the risk of someone having a fall.

Family, friends, neighbours and carers can help make

things safer for an elderly person while allowing him or her

to stay independent and self-sufficient.

Clearing away clutter, ensuring floors and carpets

are safe, making sure the home is well lit and installing

handrails where necessary are all useful suggestions for

improving safety.

Remaining active, eating well, paying attention to

foot problems, properly fitting shoes and slippers can all

contribute to avoiding falls. Taking steps to improve and

maintain health and general wellbeing is an important

aspect of falls prevention for elderly people.

In order to enable people to live in their own home for

as long as possible, a range of home care services are

available. Help is on offer from day-to-day tasks such as

cleaning, shopping and food preparation/cooking through

to personal care that helps with tasks like dressing, bathing,

toileting and prompting the taking of medication. Care

workers need to be properly trained, particularly in moving

and handling and the use of hoists for some tasks. Before

you make any decisions regarding the provision of care,

be sure to contact several providers and ask for a ‘Service

User’s Guide’ and their charges and a draft contract

between you and the care provider.

Daily care/domestic support

Care workers can call in on a daily basis to assist with any

of the tasks described above. Depending on the level of

help required their visits can be just half an hour or up to

several hours. Generally, visits are available from 7.00am

until 10.00pm. Some people will need multiple visits

per day. Night services can also be provided. The hourly

rate for these types of services depend on the services

required, the time of day and the location. Rural areas

may present particular difficulties and a travel charge will

probably be made in addition to the normal hourly rate.

Live-in care

24 hour live-in care can accommodate people with a

very high dependency on a permanent basis. It can also

provide respite breaks for regular unpaid carers and short-

term support following hospital discharge.

In some cases it’s preferable and more economic to

have a care worker actually living in the home. This can

be for a short period e.g. a week, or on an on-going basis.

Typical charges for this service depend on the amount of

care and the particular skills required. Live-in care is also

available to people with permanent physical or mental

disabilities that require long-term on-going care.

All home care providers are regulated and inspected

by the Care Quality Commission who publish inspection

reports on its website www.cqc.org.uk. When

considering a service it’s always a good idea to check its

• What experience does the agency have in your particular

• Home care agencies providing personal care, must

be registered and inspected by the Care Quality

Commission. Ask to see a copy of their registration

certifi cate.

• How long has the agency been operating?

• How many care workers would the agency assign to care

for you and would you see the same one every day? If

not, how does the staff rota operate and what happens

if your care worker goes on holiday or is sick, will you be

notifi ed in advance that a diff erent care worker will be

attending?

• How can you contact the agency in an emergency or

outside offi ce hours?

• How hard or easy would it be to make a complaint and

how are things then put right?

• If this is a private contract ask for a copy of the agency’s

contract terms and read these carefully with someone

you trust. Get as much help as you can going over it and

ask any questions you may have before signing anything.

The Citizens Advice Bureau or Age UK could help with

this.

These questions may be useful when considering using the services of a home care (domiciliary) provider to help you

build up a picture of how your care needs will be met in your own home.

People arranging and funding their own care either privately or through a Direct Payment or their Personal Budget are

able to choose their own worker or care agency. Lists of local home care providers begin on page 23.

• You should expect your personal preferences, dignity

and privacy to be respected. Discuss this with the

agency and ask how the most suitable care workers for

your particular care are chosen. Can you talk to them

before deciding?

• Care workers should be fully trained or be in on-going

training. Ask the agency about their policies on this.

• You will have a care plan drawn up by the agency which

the care workers will work to. Ask how often this plan will

be reviewed by the agency.

• Care workers must be checked with the Disclosure

and Barring Service (DBS) and have a criminal records

disclosure – make sure this is the case.

• If your care needs do not meet Adult Social Care’s

eligibility criteria and have been arranged privately you

should check carefully the fee rates charged and exactly

what the payment you make covers.

East Boro Housing Trust’s Handy Van Repair Service provides a

friendly, professional repair service, providing practical, useful

help around the home.

If you are aged 60 or over, own or rent your home in

Bournemouth or Poole and receive benefit support, e.g.

pension credit or attendance allowance you are eligible for

the service.

Contact the service on 01202 883503 and a member of

staff will be happy to help.

There is no charge for labour, no membership fees and no

hidden costs. All you pay for is the cost of materials needed

for your repair as the Handy Van Repair Service is funded

by Borough of Poole and Bournemouth Borough Councils.

Dorset

The Dorset Accessible Homes Service (DAHS), available

from 1st April 2015, will carry out small jobs around the

home. For example, small repairs, minor adaptations, home

security improvements, installation of key safes and energy

efficiency improvements.

As well as providing a handy van service, the DAHS will

be able to provide advice and information on adaptations

to your property, simple pieces of daily living equipment,

assistive technology and options for moving to an

alternative home.

Further local help

Day services are provided in a variety of locations, ranging

from residential and nursing homes to community and

specialist centres. The services aim to help people live

as independently as possible. If you have a learning or

physical disability or if you are an older person, Adult Social

Care will help to support you in finding activities which will

help promote independence.

Bournemouth Connect offers quality day opportunities,

based at local day centres, which support people to be

part of the community and use local services. Boscombe

Connect offers a generic service, and there are specialist

services for people with dementia at Northbourne

Connect, for people with mental health needs at First

Point Connect and for people with learning disabilities at

Wallisdown Heights Connect.

To be eligible for day services, Adult Social Care will

have to assess your needs. This means they will have a

discussion with you to agree with you the best way of

organising support. They will consider your needs and

decide with you what services might be suitable for you.

Dorset Partnership for Older People Programme (POPP)

Dorset POPP is primarily a partnership between Dorset

County Council, Dorset Clinical Commissioning Group,

voluntary organisations and older people.

It is a complex and ambitious person-centred

programme, led by the needs, desires, and aspirations of

Dorset’s older people.

The vision of Dorset POPP is ‘to build supportive

communities to enable older people to remain living in

their own homes for as long as they wish’ by developing

responsive, appropriate services and activities at a localised

level.

Sheltered Housing could be ideal for you if you are largely

independent, but need some help and the security of

having someone close at hand. Most sheltered housing

schemes consist of 20 to 40 self-contained flats or

bungalows, often with communal areas, such as a lounge

and garden and organised social activities. They usually

have a scheme manager who may live on site and an

alarm system to provide 24-hour emergency help.

SAIL offers residents of Dorset free support, services or

information to help keep them safe and independent

in their own home. SAIL is a Dorset-wide partnership

between local authorities, Dorset Fire and Rescue Service,

Dorset Police, voluntary organisations, local groups and

services, Dorset Energy Advice Service Centre, Age UK, and

the local health service.

SAIL can provide people with additional support,

services or information, including:

• fitting free smoke detectors and having a home safety

check;

• a home energy check and information about grants for

insulation;

• benefit checks to ensure that people are receiving

everything they are entitled to;

• debt advice;

• signposting to local social opportunities such as lunch

clubs, exercise classes or learning centres; and

• signposting to falls prevention services.

Extra Care Housing is a form of sheltered housing with

staff available 24 hours a day providing care and support.

Extra Care Housing provides you with the peace of mind

of knowing that help is at hand if you need it. It is a really

good option if you want to remain independent in the

comfort of your own home but have the benefits of help

at hand if you need it.

You pay for the care you want separately, as and when

you need it.

Extra Care Housing offers many facilities, often including

a dining room, restaurant, communal areas, laundry,

gardens, guest facilities and social activities.

Extra Care Housing is available to anyone over the age

of fifty who is in need of extra care and support to remain

independent.

Sheltered Housing and Extra Care Housing are available

to rent from private landlords, Housing Associations or

Poole

Currently Poole has 80 social rent units across two

schemes: Delphis Court and Belmont Court.

Delphis Court in Poole Town Centre is a purpose-built

development consisting of 45 one- and two-bedroom

apartments. These are spacious independent homes

with their own front door, within a community, that are

designed to meet the needs of both active and less active

people who require care and support associated with

ageing or those with lifelong disabilities.

Personalised packages of care can be delivered to suit

the needs of the tenant; these can be as extensive as

needed. In addition there is care and support available

in emergencies 24-hours a day with an on-site waking

night carer available seven days a week. There is a Scheme

Manager on site 9.00am to 5.00pm Monday to Friday.

All private and communal areas have pull cords linked

to a care line to ensure residents' safety at all times. The

development is managed by Synergy Housing.

Belmont Court, Parkstone is managed by Poole Housing

Partnership. It consists of 35 one-bedroom flats and one

two-bedroom bungalow. All the flats and the bungalow

have Lifeline equipment installed. It has a full-time

Sheltered Housing Coordinator to deliver housing-related

support from 9.00am to 5.00pm, evening waking care

cover from 5.00pm to 10.00pm and night sleeping care

cover from 10.00pm to 9.00am, 365 days a year.

It is in an accessible location with bus stops within easy

walking distance, and a good shopping area nearby. There

is a meal service that operates from the kitchen within the

scheme to provide affordable and healthy hot meals for

residents. There are regular day time and evening activities

for the residents of Belmont Court to participate in.

A further scheme is currently being developed with

54 new extra care apartments with communal facilities

available at Trinidad House, Rossmore Road in 2016.

Eligibility for the schemes is via the Dorset Home Choice

system. All vacancies are advertised through this system,

therefore suitable applicants ideally need to be on the

Housing Register. An allocations panel of representatives

from Housing and Community Services and Adult Social

Care consider applicants who want to be offered a

property on these developments. They follow the Local

Lettings Plan which sets out the eligibility for the scheme

and how applicants will be prioritised.

In addition there is some private leasehold assisted

living accommodation in the Borough which although

uses different terminology, has the characteristics of Extra

Care Housing.

or mental health issues. There is also a range of housing

options you could explore.

Physical disability

Specialist care services

The support you require if you have a physical disability

must be tailored to your specific needs and can be

provided by the independent and not for profit sectors as

well as by Adult Social Care. Help at home with personal

care is available according to need, during the day and

night, seven days a week. There are also private agencies

offering help at home with personal care and housework.

Services from Adult Social Care include: occupational

therapists; help with adapting your home; the

‘Handyperson Scheme’; activities within the community;

help at home, with meals and leaving hospital; respite care;

supported housing, and help for carers.

In Dorset, the Dorset Accessible Homes Service (DAHS,

available from 1st April 2015), will provide a range of

services aimed at supporting you to maintain your

independence in your own home. This will include advice

and information, support to consider a move to alternative

accommodation, assistive technology including telecare,

minor repairs, home safety, security, energy efficiency

improvements and minor and major adaptations. There

will be a Dorset Accessible Homes Centre including a

demonstration area, offering advice and ideas to support

> in the community? What are the home’s links with

specialist staff in the community services? Use the care

homes checklist on page 41 when visiting potential homes

which includes helpful questions to ask.

Are you happy living at home? Would you like a little

more independence with help when you need it? What

are your housing options? You could:

• Continue living with your family – If your home is

specially adapted and you receive the practical and

emotional support needed, you may prefer to stay at

home. It’s also convenient if your college or job is nearby.

• Move into supported living accommodation – This

offers a level of independence and the chance to live

alone or meet other people in similar situations. Help

from a visiting care assistant is available and usually

nursing care is provided nearby.

• Rent a place – You can apply for council housing which

is usually cheaper than renting privately. There could be

a waiting list and you must fully explain your housing

needs. Local housing associations may also be able to

help. Ask your council housing department for a list. If

you want to rent privately, look in your local newspaper

for ‘To Let’ advertisements. Housing Benefit, if you’re

entitled to it, might reduce your rent. Direct Payments

can also help, ask your local authority for details and see

page 8.

• Buy your own home – Can you afford it? Are you able

to live on your own? You could think about ‘part buying’

and ‘part renting’ a place from a housing association.

• Move into a care home – If you want to leave home

but are not ready to live alone, care homes can meet

your personal needs and offer appropriate support. Adult

Social Care and Health will contribute if you are eligible

and if they agree the care home meets your needs. If

your chosen home costs more than usual, you may get

some help but you must pay the rest.

When living independently you may require some

adaptations around the house. You can choose housing

that’s specially adapted. There are organisations that

specialise in providing this sort of accommodation

including Livability (formerly John Grooms) and the

Accessible Housing Register.

New homes that have been built since 1999 should be

fully accessible to disabled people. However, if you choose

an older home, there are a number of adaptations that

could make it easier for you to use. These include wider

doorways; a downstairs toilet; bathroom equipment

including handrails, a walk-in bath, electronic bath seat;

kitchen adaptations including lower working surfaces;

lower level light switches; easy-to-use controls for heating;

a stairlift; security intercom at the front door and ramp

access to the garden. You should also think about the

general access to your home.

Learning disability

Learning disability services are run by a partnership

between Adult Social Care and the NHS. Community

learning disability teams are the first point of contact for

advice, information and guidance on services for people

with a learning disability. You can contact them through

your local Adult Social Care office, listed on page 4. You

can also ask your local office for copies of factsheets which

tell you more about day care services, care home provision

and services for carers available locally.

Adult Social Care provides care where necessary and

gives support to enable and encourage service users to

reach their full potential in all areas of their lives. Access to

services is via an assessment, explained further on page 7.

People with learning disabilities can also use a Personal

Budget (explained on page 8), which puts people at the

centre of how they choose to pay for their support. This

may be in their own flat or house or a supported living

scheme (see page 34).

Your local Community Mental Health Team (CMHT) should

be your first point of contact if you have mental health

issues. You can ask your GP to refer you or ask someone

who knows you to contact the CMHT on your behalf. The

team will discuss your housing choices with you.

If you have mental health needs, residential care can

help improve the quality of your life - the structured

environment and activities provided can build up your

confidence. The care providers should assess you carefully

to make sure the placement is right for you. A qualified

member of the residential care staff may visit you at home,

talk to you and look at your assessments of health/social

care need.

To find out more about mental healthcare in your

region, organise visits to prospective care homes. Ask

the residents about the care they receive. Are psychiatric

nurses available if needed? Do the activities provided assist

rehabilitation? What are the attitudes to medication and

other therapies?

The term dementia describes a collection of symptoms,

including a decline in memory, reasoning and

communication skills, mood changes, and a gradual loss of

skills needed to carry out daily activities.

Symptoms are caused by changes in the brain due to

certain conditions like Alzheimer's disease and strokes.

Dementia can affect people of any age, but is most

common in older people. Dementia is a progressive

condition. It can start with small changes such as memory

loss (although becoming forgetful is not necessarily a

symptom of dementia). The rate of progression varies and

each person will experience dementia differently.

Diagnosis First steps - see your GP

Many people experiencing memory loss and dementia

become isolated from those around them and from

sources of help and support. It is therefore essential that if

you, or someone close to you, has symptoms of memory

loss or dementia, you seek a proper diagnosis from a GP.

Memory Support and Advisory Service

Since September 2014, the Alzheimer’s Society provide

a Memory Support and Advisory Service across Dorset.

Memory Advisers will provide expert practical guidance

and support to people affected by dementia or people

concerned about their memory.

Dementia-Friendly Communities

In March 2012, the Prime Minister’s Challenge on Dementia

was launched which raised ambitions in relation to

supporting people to ‘Live well with Dementia’ particularly

focusing on how society accepts and supports people to

live positively in their local communities. The Challenge

stated that ‘lack of awareness among the public and poor

understanding in communities has a major impact on the

experience of people with dementia.’

Poole Dementia Action Group (PDAG) was formed in

September 2013. The main objective was to make Poole

High Street dementia-friendly. The following webpage

shows which companies on the High Street have signed

an action plan and what actions they will put into place to

become dementia-friendly.

Support for carers

Caring for someone with memory loss and dementia

brings its own challenges. The first of these is often

helping them seek a proper diagnosis. Even without a

diagnosis, there is a wide range of advice and support

for carers.

A Carers in Crisis support scheme is available for carers

in Bournemouth and Poole, explained on the website


Many people living with dementia, and their carers,

are happier remaining at home, maintaining their

independence in a familiar environment.


Alternatives to staying at home

When living at home is no longer possible, there

are a number of options to consider, from sheltered

accommodation, through extra care sheltered housing to

care homes.

This Is Me - My Care Passport is useful when receiving

any care or treatment - it gives health and care staff

important personal information about you, and helps

them care for you as you would like. Ask about this when

you are admitted.

The ‘Memory Support and Advisory Service’ is provided

by the Alzheimer’s Society and works with GPs and

the Dorset Healthcare Memory Assessment Service.

The service has a team of advisers who are trained and

experienced in dementia issues. They help people stay

independent and support those living with dementia by

providing information, advice and guidance.

You can be referred to the Memory Support and

Advisory Service by your GP or a number of other

agencies.

Memory cafés

Memory cafés are places where people can go with their

carers to just socialise and have fun. You can relax and be

yourself, knowing that no one is judging you and everyone

is going through almost the same things as you every

day. A memory café has no real agenda or stated purpose

except to enjoy each other's company: they are a place

for mutual support and information sharing perhaps with

reminiscence-based activities.

Residential dementia care

If the time comes when remaining at home is no longer

viable, the benefits of residential care need considering.

Questions to ask of any care home registered for someone

with dementia would include: who is the senior clinician

who can act as a single point of contact for people to

approach for information and advice? What training

programmes do staff participate in? Does the home offer a

person-centred approach to its care planning? Is the home

designed with the needs of its clients in mind, especially

safety and accessibility? Is there plenty of natural light?

Is moving around helped by easily-understood signage?

How is technology employed to help alleviate falls and

help orientation? Please also see the care homes checklist

on page 41 and the residential dementia care checklist on

page 38.

Differentiating one home from another can be difficult,

but the quality and quantity of staff training is especially

important, along with the turnover of staff, as continuity of

relationships in this type of home is critical.

The use of medication is a highly emotive subject

and any antipsychotic medication must be very carefully

prescribed. A fine balance exists between ‘acting in the

best interests of the individual’ by prescribing medicine

and the benefits of ‘normalisation’ where time and energy

are spent maintaining familiar, comfortable surroundings

which help reduce stress and possibly avoid the need for

certain medication.

Residents

The best indication of a good home is that the residents

appear happy and responsive.

• Are there rummage boxes around the home to stimulate

residents?

Access

If the person with dementia needs or is likely to need

equipment or adaptations you may want to check:

• There is adequate signage and cues for diff erent parts of

the home such as dining room and bathroom.

• The home’s policy about locking external doors.

Bedrooms

You may want to fi nd out whether the person with

dementia can have a single room and whether:

• Residents are encouraged to bring in some of their own

furniture and possessions to increase familiarity.

Activities

Residents should be stimulated without feeling stressed.

• Are reminiscence activities available?

• Are residents able to compile memory boxes?

• Do staff compile life story books including photographs

and mementoes?

Staff

It is important to note whether staff seem friendly and

caring towards residents and whether they treat residents

with respect.

• Do they have any training and experience in dementia

care?

• Will the person with dementia have a member of staff

particularly responsible for their care?

• Is there a member of staff you can talk to about your own

worries about the person with dementia?

Manager/head of home

A manager who is caring as well as effi cient can make all the

diff erence to a home.

• Does the manager have knowledge of dementia and

can they deal with diffi culties that may arise in an

understanding way?

• Is there a full assessment at home before a resident is

admitted?

The Life You Want. The Care You Need.


Care homes may be an option if you can't manage at

home and other housing options aren't right for you. You

will be looked after by trained staff, in your own room.

However, they are not the right choice for everyone. You

will get support and companionship, but you will give up

some independence. There will be compromises about

what you can do and when, compared to living in your

own home.

As well as thinking about whether or not a care home

is right for you, you will also want to consider the cost.

A residential care home (without nursing care) can cost

anything from £500 to £1,400 a week, i.e. from £26,000 to

nearly £73,000 a year.

Adult Social Care has found that many people go into a

care home before they need to. This means that they can

spend more money, often life savings, than they need to.

If you move into a care home funded from your savings,

there may come a time when you can no longer afford to

stay there and you may need help from Adult Social Care.

If this happens, Adult Social Care can help but we have a

limit on how much we will pay for a care home place. This

means that you might not be able to continue to stay in

the care home of your choice.

It is clear that moving into a care home is a big decision.

If you still think it might be right for you, it is best to talk to

your local Adult Social Care department first. They will talk

to you about your needs and give independent advice on

the best choice for you.

All care providers in the country must be registered

with the Care Quality Commission (CQC). All services are

inspected by the CQC, who report on their findings. These

inspection reports are available from the care service or

the CQC (visit: www.cqc.org.uk). Further information

about the CQC can be found on page 46.

Types of care home

Care homes

There are two types of care home:

Care homes offering personal care only

If you are reasonably active, but would like greater security

and care, subject to a needs assessment, a care home

offering only personal care may be the best option.

Personal care includes bathing, feeding, dressing and help

with moving and must be paid for in full if your capital/

savings exceed £23,250. If you have less than this amount,

Adult Social Care may be able to assist with funding if they

agree living in a care home is the best way of meeting

your needs.

Care homes with nursing

If you think you may need nursing care in a home, you will

need to be visited by a social worker or a care manager

to work out what care you will need. This visit might be

in your own home, or in hospital if you’ve been ill, or in a

care home. You will be fully involved in planning your care

needs.

If a care home providing nursing care is the best

solution for you, the social worker will give you information

to help you find a home which meets your care

requirements. The cost of the nursing care part of your

fees is paid by the NHS to the home directly: the current

amount is £110.89 per week. You can contact the Nurse

Care Management Teams for your area at your local Clinical

Commissioning Group listed on page 51.

Care homes checklist


Even if you know you will have to pay for it yourself, if

you need care in your own home or are considering

moving into residential care, you still qualify for a social

care assessment by Adult Social Care. The assessment

will make sure that your needs will be met. Also, if you

expect your capital to fall below £23,250 as a result of

paying for care, Adult Social Care may then help you with

the cost.

If you are moving into residential care, you may be

entitled to some of the following financial assistance

and support:

Twelve-week property disregard

This applies if:

• your former home is included in your financial

assessment,

• your other capital is less than £23,250, and

• your income is not enough to meet your care home fees.

Adult Social Care will help with the cost during the first

twelve weeks of permanent residential care, provided the

assessment has shown that this is the kind of care you

need. This is called the ‘property disregard’ period.

Did you know you can choose a care home outside the area

you currently live in? You may want to be closer to friends,

family members or you may want to relocate to another

part of the country.

If your care home place is state-funded, your local

authority is still responsible for your care fees, even if you

choose a care home in another area.

The home you choose must be suitable for your assessed

needs and comply with the terms and conditions set by

the authority. The fees that your local authority will pay may

vary. They may offer you the fees you would receive if you

remained where you are, or they may offer you the fees that

the local authority would pay in your chosen area. You must

seek further advice before making your decision.

If you’re self-funded, obviously you have freedom of

choice. However it is important that you have considered

if the local authority will agree that you meet the eligibility

criteria for a care home placement if your savings fall below

the capital limit. What are the home’s fees and will it accept

the local authority rates if there comes a time that your

savings fall below the capital limit? If the local authority will

not fund the placement or the home is unable to meet

the local authority rate you need to bear in mind that the

possibility of moving to an alternative home increases

substantially.

Whatever your care needs, this Directory, the Care

Choices helpline and website will be able to point you

in the best direction, however you would like care

information presented.


Deferred payment agreements

After the twelve-week property disregard period, Adult

Social Care may be able to help you while your property

is being sold. In these circumstances they will charge any

financial help they give against the value of your home.

This means they will take back the amount they have paid

once your property has been sold. However, they may

limit how much they will pay.

It may affect your entitlement to Pension Credit if your

property is not seen to be on the market and becomes

treated as capital by the Department for Work and

Pensions (DWP).

What if you run out of money?

If your capital is likely to reduce to £23,250 as a result of

paying for care, you must tell Adult Social Care well in

advance that this is going to happen. They will then be

able to help with your care fees (provided your care needs

assessment has shown that you need residential care).

They will carry out a financial assessment to confirm the

date from which your funds fell below £23,250, and the

amount they, and you, will pay towards your care.

If the home you have chosen charges more than

the local authority will pay, you must find someone to

pay the difference – a ‘top-up’ payment. Whoever does

this, whether a family member or a charity, they should

understand that the cost could increase, and that they will

have to pay the top-up for as long as necessary. The local

authority will not meet the additional cost even if you have

lived in the home for a number of years. If you cannot find

someone to help you meet the extra cost, you may have

to move to a home within the local authority’s funding

levels. For more information, see page 44.

Understanding your rights before moving into care

is essential. There are a number of financial products

and specialist companies who may be able to help. It is

important to seek financial and legal advice about the

various options you may have before committing yourself.

Attendance Allowance

This is a non-means-tested, non-taxable benefit from the

DWP. It’s paid at the lower rate of £54.45 a week if you

need care by day or night and at the higher rate of £81.30 a

week if you need care during the day and night. Everyone

who needs care can, and should, claim Attendance

Allowance. If you are paying the full cost of residential care,

with or without nursing care, you will be entitled to the

higher rate.

NHS Nursing Care Contribution

Whether you are a temporary or permanent resident, if

you live in a care home that provides nursing care you may

be entitled to NHS Nursing Care Contribution towards the

cost. It is not means-tested and is currently £110.89 a week.

This is paid directly to the home.

Do you have more than £23,250 in capital and

savings?

The value of your home is included in the assessment of

your capital unless one of the following still lives there:

• Your partner (that is, your husband, wife or civil partner, or

someone you live with as if they were your husband, wife

or civil partner);

or

What will you have to pay for your care?

Paying for your own care continued

• A relative who is over 60 or disabled;

or

• A child under 16 who you, or a former partner, are

responsible for.

IF NO

Adult Social Care will contribute financially towards your

care costs after they have carried out an assessment of

your care needs.


How much?

You will need to ask Adult Social Care for an assessment.

This will define your care needs and how they will be

best met. Any care homes you consider must be able

to meet these needs. Adult Social Care will also make an

assessment of your financial situation. This determines

how much they will pay and how much you will have

to pay.

Your choice of care home will be limited to those that

accept your local authority’s funding level. If you want

to choose a more expensive home you would have to

arrange for a third party - such as a family member or

charity - to ‘top-up’ the difference. You are not allowed to

do this yourself if your capital is below £23,250.

If you have capital between £14,250 and £23,250 you

will pay £1 a week for every £250 you have above £14,250.

If your capital is less than £14,250, and the home you

choose charges fees that are within your local authority’s

funding rate, your contribution will be assessed only on

your income.

IF YES

Ask your local Adult Social Care department for an

assessment. See contact details adjacent.

Any care homes you consider must be able to meet

your assessed care needs.

If, apart from your property, your savings are less than

£23,250, Adult Social Care can help with your care costs for

the first twelve weeks. After this time you can apply for a

loan to help you whilst your property is being sold.

WHATEVER YOUR CIRCUMSTANCES:

Remember:

• It is just your own financial circumstances that are

assessed, not your partner’s.

• Your assessment will be made up of two elements; a care

needs assessment and a financial one.

• A care home with nursing will generally be more

expensive than a care home.

Consider claiming:

• Income Support

• Pension Credit

• Savings Credit (if you are over 65)

Definitely claim:

• Attendance Allowance, worth either £54.45 or £81.30 a

week depending on your care needs.

Moving into a nursing home?

Third party payments for care home fees

Some care homes and care homes with nursing charge

fees that are higher than the maximum amount Adult

Social Care can contribute. If Adult Social Care are

contributing towards your care home fees and you

choose to move into a home which charges a higher

fee, the difference between the two amounts is paid by

a third party, usually a relative. Third party payments are

sometimes referred to as ‘top-up’ payments.

Before you sign any contract, the home should give you

written details of all the charges it intends to make in its

‘Statement of Purpose’ document. If there is anything that

is not clear or which you do not understand, you should

ask for advice.

As the name suggests, this payment must be made by

someone other than yourself or Adult Social Care. It can

be a relative or friend, but it cannot be a ‘liable relative’ i.e.

someone whose financial situation has been taken into

account when your financial assessment was carried out

by Adult Social Care.

The general rule is that you cannot use your own

money to fund a third party payment.

However in certain specific circumstances you may

make a third party payment from your own funds. These

are:

• when you are eligible for the twelve-week property

disregard (see page 42) or

• where Adult Social Care have agreed to a deferred

payment until your home is sold (see page 43).

Adult Social Care recommends that you seek

independent financial and legal advice if you are

considering these options. If you need further advice

about how a top up to your care home fees might be

arranged please discuss this with your care manager.

If you move to a care home where a third party

payment is required, the person who will make the

payment on your behalf must sign a contract with Adult

Social Care, before the contract with the home is signed.

In doing so they must confirm that they have the financial

means to make the third party payments (including any

future increases) for the whole time you will live at the care

home.

If you are already a resident in a care home, and no

third party agreement was required at the time you

became a resident, the home may seek to introduce a

third party payment at a later date. Also if a change to

your arrangements is made at your request or with your

agreement, for example you move to a superior room,

then an additional third party payment can become due.

It is usual for the fees charged by care homes to increase

each year. If you are not paying your own fees, the amount

that Adult Social Care can contribute may not increase

at the same rate as the home fees increase. Any shortfall

in fees must be met by a third party (top-up) payment –

discussed above. The third party element of the fee can only

be increased once in any financial year and as a guide, the

amount should never increase by more than 25% a year.

You should receive at least one month’s notice that the third

party payment is to increase. The person who makes the

payment for you must sign a new contract with Adult Social

Care if the amount of the third party payment increases.

If you are already living in a care home, plan ahead and

find out what help you will get from Adult Social Care when

your savings fall below the £23,250 limit, explained on

page 43.

All care providers must be registered under a system

introduced by the Health and Social Care Act 2008, which

brings adult social care, independent healthcare and the

NHS under a single set of essential standards of quality and

safety for the first time.

The Care Quality Commission (CQC) is the independent

regulator of health and social care in England. It regulates

care provided through a range of methods including:

• registering providers to ensure they are meeting the

essential standards of quality and safety;

• monitoring how providers comply with the standards;

• promoting improvement in services;

• using enforcement powers where necessary;

• publishing up to date information about the compliance

of adult social care providers with essential standards of

safety and quality.

From October 2014, following an inspection, each provider

is given a quality rating on how it rates against national

essential standards of quality and safety. Each service’s

Comments, compliments and complaints

All organisations need to know how they are performing.

They are happy to receive your feedback on their service

whether they are compliments or complaints. Feel free to

tell them what you think and your comments can be used

constructively to improve the service.

If you do need to make a complaint you should feel

able to complain about any aspect of your care that

affects your happiness or comfort. This could be about

the way you are treated by a staff member or the quality

of the food you are served. You can also make comments

and suggestions about possible improvements to your

Making a complaint should not be difficult for you.

Providers are required under essential standards of quality

and safety to have a simple and easy to use complaints

procedure that they will be happy to give you. If you are

concerned about the care that you, a friend or a relative is

receiving, it makes sense to speak to the manager before

you take any further action. The problem may be resolved

quite easily once they are made aware of it.

However, if you need to make a formal complaint,

you should initially contact the registered owners of the

service. They have a duty to respond to any complaints

made.

another option is to complain to your social worker/care

manager or the Complaints Manager in your local council:

Who is at risk of abuse?

People may be at risk of abuse if they:

• depend on other people for their care;

• are older, frail and unable to protect themselves;

• have mental health problems;

• have a physical or learning disability;

• have a serious sight or hearing impairment;

• have dementia;

• misuse alcohol or drugs; or

• have a long-term illness.

What does abuse mean?

Abuse takes many forms. It includes:

• any form of harm such as:

- hitting, injuring or restraining;

- threatening, intimidating or humiliating;

- sexual attention or activity that is not wanted; or

- keeping someone on their own.

• any form of neglect such as:

- not giving the correct medicine;

- not providing food or clothing; or

- not arranging the right care.

• financial abuse such as:

- stealing or misusing money or property; or

- pressure about wills or inheritance.

• discrimination, i.e. treating someone less favourably

because of race, ethnicity, religion, age, gender, disability

or sexual orientation.

Abuse can happen anywhere - at home, in a care home,

hospital or day centre or in public.

Someone tells you about abuse

If someone tells you they are being abused:

• stay calm and listen to them;

• offer them support;

• write down what they tell you using their own words;

• keep any evidence safe;

• get in touch with us or the police; or

• in an emergency dial 999.

Please do not:

• press them for more details;

• contact the alleged abuser;

• promise to keep it secret; or

• assume that someone else knows what's going on and

will get in touch.

All public services, police, local authorities and the NHS,

take adult abuse very seriously - we usually refer to this as

‘Adult safeguarding’. However, adult protection from

abuse is more effective if we all take responsibility for

people at risk.


Reporting abuse

If you know someone is being abused, or think they may

be, call:

If it is outside of normal office hours, please call the Out of

If you think someone may be at immediate risk of harm

You can also tell a health or social care worker such as a

social worker, nurse, doctor or occupational therapist.

If you report abuse to Adult Social Care, they will:

• take your concerns seriously;

• deal with the matter sensitively;

• make sure the person is safe; and

• investigate the situation fully.

Adult Social Care will ask you to provide a name and

contact details in case they need to get in touch to ensure

they have all the information they need to keep the

person safe.

nursing – whether long-term or for

short-stay respite, convalescence,

or rehabilitation.

The nursing home of today is a

far cry from the outdated perception of latter years. Today’s

nursing home combines an ambience of the client’s

personal home, with quality furnishings and surroundings,

together with a warm caring atmosphere backed up by

qualified, professional nursing staff. A total package of care

is provided, designed exclusively for the individual, which

encompasses social, medical and welfare needs.

It may be that you are making the choice, or more likely

nowadays, a family member or close friend is looking on

your behalf.

There is one level of comfort that can go a long way to

removing some of the concerns of placement – choosing

a ‘Blue Cross’ home.

What should you expect from a quality ‘Blue Cross’

home?

• 24-hour nursing services provided by a team led by a

qualified nurse.

• Accommodation furnished and equipped to provide

comfortable ‘homely’ surroundings. Most homes have

lifts, lounges, dining rooms and gardens/conservatories.

• General welfare, social and occupational therapy activities

designed to enhance the client’s quality of life.

• Compassionate, palliative, and End of Life care working

with the client, the relatives, and under professional

medical direction.

The most important consideration from your

perspective is that the nursing home provides a holistic

approach to care – all aspects of health are taken into

consideration and the use of NHS or GP resources ensures

prompt action. If you are frail or elderly, detecting health

problems, and responding early, stops you suffering

unnecessary pain or discomfort and prevents avoidable

traumatic admission to hospital.

It would be nice to think that your retirement and old

age will be worry free. It can be. Advance thought and

preparation can pay dividends for you and your loved ones

at a time when you may be less able to cope by yourself.

Some people are lucky enough never to need help with

their fi nancial or personal arrangements. For many,

though, health issues or general slowing down in old age

mean that decisions and action on day to day matters

(which seem routine when you were younger and fi tter)

need to be taken by someone else on your behalf.

If you take no steps to make arrangements yourself

then the only course of action may be the appointment

of a Deputy through the Court of Protection. The Court

will oversee the actions of your Deputy and complicated

work is necessary to comply with the Court’s

requirements. A level of formality is needed which can

seem harsh and intrusive, not to mention expensive.

The alternative is for you to make decisions yourself

ahead of the diffi cult times which you may face in the

future in the form of a Lasting Power of Attorney (LPA)

for either your fi nancial or welfare aff airs or both. An

LPA enables you to appoint people of your choice to

handle your aff airs and gives your personal authority to

them to act with greater ease on your behalf.

It is important to note that if you have already made

an Enduring Power of Attorney it will probably still be

eff ective, but it would be sensible to take advice on

whether a welfare LPA would suit your future needs.

A new style of care home has just opened in Poole. It is the creation of Sarah Jessup, who, with her parents, has operated Burwood Nursing Home in Broadstone for over 30 years.

The new concept accommodates residents with varying degrees of nursing care and the design incorporates these needs not only from a medical angle, but also from an environmental perspective. It is also important that the home does not feel like an institution or hospital, so the home only has 42 rooms with lots of communal space; it can be described as a boutique care home.

When your family member or friend moves to a care home they need to maintain a sense of independence. That means choosing to do what they would like to do and where to go and at Yaffle this is possible without leaving the comfort and safety of the home. However, should a resident choose to spend the afternoon in their room, then there is the benefit of enjoying the views of the lovely wooded surroundings facilitated by the private balconies and patios.

An important aspect is stimulation, variety and it has to be enjoyable and fun. You can choose to go with your family

to the Railway Tavern for a game of cards and a ploughman’s lunch or perhaps bake some scones in the resident’s kitchen.

Dining is also an important daily occasion. Traditionally lunch at a care home arrives in the dining room on a trolley. Where has it come from, who cooked it, what is it? At Yaffle the open plan dining room means that residents can see their food being prepared, cooked and served and can also let the chef know what they prefer and how they like it cooked.

The ability to enjoy the wildlife and setting of the new home is also an important feature that has been incorporated in to the design. Large outdoor terraces allow southerly views over Yaffle Hill towards the Purbecks. All bedrooms are attractively furnished with modern comforts such as an ensuite wetroom, television with internet access, electric profiling bed and ceiling hoists to facilitate the safe and comfortable movement of less mobile residents.

Tender Care provides care to adults (over 18 years), Mental Health, Physical Disabilities, Learning Difficulties, Alzheimer’s Disease and Dementia, Older People and Sensory Impairment. We offer Palliative Care by a small and select team who are experienced in caring for those through the end of life process.

We provide domiciliary care of the highest standard to adults within their own homes. We encourage independence whilst maintaining respect and dignity enabling them to remain at home.

Respite Care

We provide a service to those who rely on relatives or friends for their care but would like to give that person a break from time to time.

Most people would like to get out and about occasionally and would like someone to take them out for a couple of hours or for assisted shopping outings. We offer a sit-in service for afternoon companionship.
