Must meet Exemption Form requirements even if an above listed employment type

1) Complete top of CBC form2) Give CBC form to applicant at interview

or immediately when you know you would like to hire

3) Employee completes and returns to Erica in BATO directly (confidential fax, email, envelope)

4) BATO processes through UWM Police Dept

5) Email sent to hiring department with outcome

6) Employment offer or Letter of Offer
