Compassionate Communication: Tips for Dementia Caregiversby Ava M. Stinnett

Communication mix-ups. They happen all the time, don’t they? Some of the more humorous mix-upsinvolve misheard song lyrics. Remember “Dancing Queen” by the group ABBA? The lyric “Feel the beatof the tambourine” has often been misheard as “Feel the beat of the tangerine.” Who isn’t amused whena toddler refers to strawberries as straw babies or pronounces grilled cheese as girl cheese? Morerecently, the autocorrect feature on cell phones has provided some noteworthy examples ofmiscommunication, as has the misuse of Internet acronyms. One woman, for example, texted variousfriends and family members to let them know about the passing of a beloved family member. Sheincluded the acronym LOL at the end of the text, thinking it meant “lots of love.” She was mortified whenher son told her it actually stands for “laughing out loud.”

In dementia-related diseases such as Alzheimer’s, communication mix-ups may start slowly withdifficulty finding the right word—a person’s name, for example—or incorrectly substituting one word foranother. Persons living with Alzheimer’s may invent entirely new words to describe familiar objects orrepeat a question over and over. As the disease progresses, communication becomes more difficult.They may struggle to organize words logically or may rely on gestures. They may not be able tounderstand what you are saying or only be able to grasp part of it. Writing and reading skills may alsodeteriorate.

Losing the ability to communicate can be one of the most frustrating and challenging problems faced bypeople with dementia-related diseases, their family members, and caregivers. It’s important to keep inmind that each person is unique and that difficulties in communication are very individual. Although theymay not understand what is being said, feelings such as joy, anger, sadness, fear, and love remain.Communicating with compassion will allow them to maintain their dignity and self-esteem.Consider these communication tips:

Keep it simple.•Use short sentences and plain words, asking only one question at a time. Yes-no questions may work best.

· Be patient and allow plenty of time for what you have said to be understood.· It may take longer for your loved one to process and respond. Don’t interrupt or finish their

sentences. Just listen and avoid rushing and correcting.· Maintain eye contact and stay nearby so that they will know that you’re listening and trying to un-

derstand. Where appropriate, use touch to keep the person’s attention and to communicate feel-ings of warmth and affection.

· Try to avoid distractions and a background of competing noises such as TV or radio.· Remain calm and talk in a gentle, straightforward manner. The tone of your voice can send a

clearer message than what you actually say.Don’t argue.•Be aware that one’s reasoning and judgment will decline over time. If necessary,leave the room to avoid confrontation.

Communication may be challenging, especially as the disease progresses. Whether a family member ora caregiver, it’s critical to remember not to take it personally if the person with dementia becomesparanoid or accusatory. Patience, understanding, and 100 percent forgiveness are at the heart ofcompassionate communication.

Dear Residents, Families and Friends,What a wonderful summer we have been having! I hope you all

enjoyed our annual Family Independence BBQ on the 1st of July. Ithink the fireworks show was one of our best. I would like to take thistime to thank our fabulous staff for an absolutely wonderful evening.It was once again a very fun night with great food, entertainment,dancing and fireworks, the perfect end to a wonderful night!

I would like to remind you all that our Hot August Nights isright around the corner. It will be Thursday, August 11th. In truefashion we will have entertainment, tons of cars and of course awonderful dinner. We hope to see you all for our monthly brunchon Sunday, August 21st. We enjoy having our family members,friends and guests join us for our events. So remember to make yourreservations in advance with the front desk or by calling 971-4800.This way we will make sure we have plenty of food for everybody!

The official decision has been made to remove our salad bar. Aswe talked about at the beginning of this year the salad bar will beremoved for the health and well being of our residents, staff andguests. You will still be able to get a salad everyday at lunch anddinner. We will continue to have a weekly special salad and will alsooffer different items weekly that you can order to create your own.However, I do have some great news! I have heard that our carpet forthe second and third floors should be installed as early as September.I will keep you posted as soon as I hear that it has arrived.

As most of you know by now Marybel Caro-Parker waspromoted last month to our Enhanced Community, as the Director ofResident Services. She had been the Memory Care Manager forthe last two years and did an exceptional job. She is dedicated to herresidents and families. I am so excited to see what she can offerEnhanced. Her dedication and compassion for the residents is like noother. She is looking forward to working with all of the residents andfamilies at Enhanced. With Marybel's promotion to Enhanced, leftan opening in Memory Care. Please help me welcome back, ournew Memory Care Manager, Jessica Beck. Some of you willremember her from a few years ago as Jessica was one of Marilyn'sInterns from Sacramento State. She has been working the last coupleof years in our Davis community. We are all excited to have herback in Sacramento. Jessica is also dedicated to her residents andfamilies and brings with her much experience to the unit.

I hope you continue to enjoy your summer, eat lots of ice cream,enjoy your family and friends and take a moment to be thankful forall that is good in your life. As I have said before, I am truly blessedand thankful to come to work with all of you everyday.

Dining Scarves

Maintaining a residents dignity has always been at the forefrontof our Memory Care dining experience, and Carlton has found anew way to elevate it to the next level.

As dementia progresses many residents experience tremors or alack of coordination, causing food and drinks to be spilled on hisor her clothing. In the past we used cloth protective coverings asa barrier between residents clothing and food. Recently we havebegun to offer residents dining scarves as a dignified alternative.

The scarves are made from a chicmaterial that allows liquids to beadand run down, gathering on theresidents napkin placed in his or herlap. The scarves come in differentcolors so residents can wear themas a fashion accessory, whichhappens to serve a greater purpose.The ideas is that other residents,family, and guests will look

through the dining room without seeing protective covers, becausethe scarves are a part of the residents outfit.

You know how I love to hear about people who have learned to use their minds to improve their livesand overcome obstacles. One of my favorite true stories is about a remarkable man who overcamewhat many of us would consider one of the most debilitating of physical handicaps. I’ve written abouthim before and if you’ve taken part in the weekly Power Groups, you will be familiar with TonyMelendez. Like me, you may have wondered about his present life.

Tony was born in Chino, California, the same town where I grew up, and attended the same church,Our Lady of Guadalupe. I didn’t know him because he is much younger than I. He was born inNicaragua then moved to the U.S. when he was a year old. His father died early on. Tony’s motherhad taken thalidomide, a drug to calm morning sickness, which we now know causes birth defects.Tony was born without arms.

Having no arms didn’t stop Tony from accomplishing his musical dream, however. He started playingthe guitar at the little church in Chino, then his big break came right after he played “Never be theSame” for Pope John Paul II, during his Los Angeles papal visit in 1987. And as many of you mayhave seen the T.V. ceremonies celebrating Pope John Paul’s life which included a rerun of thatmoment, the Pope embraced and kissed Tony on the cheek; he told Tony to

National Television aired it all and that is the way the world first of heard of Tony Melendez, theremarkable guitarist who has no fingers, hands, or arms. However, he was blessed with feet and toes,and it was with those toes that he played the guitar for The Pope. He uses his right foot for pickingand his left foot for the fretting on his 12 string instrument. His musical album debut came inMarch of that year at the age of twenty-six. He specialized in love ballads and gospel. Today Tonyis married, has children and lives in Branson, Mo., where he performs.

Tony is an excellent example of what the empowered living philosophy embraces. The mind is apowerful tool, waiting to carry out our commands. Every day we give our minds the commands thatwill determine what kind of day we will have and what kind of life we will live. Here are a fewguidelines.

1. Check your thoughts the first thing in the morning and be sure they are positive.2. Only allow positive, life giving thoughts to exist in your mind.3. Surround yourself with positive, upbeat people4. Read and watch uplifting, mind developing material5. Be hopeful but take action; don’t sit back and wait for something to happen automatically.6. Develop a love for people, smile and be friendly7. Always go to bed with good thoughts
