Future ASR parents and athletes,

For me, all-star cheerleading is more than just banners and trophies or flip-flops and fulls, it ismy passion in life. I was a football player in high school and never imagined that competitivecheerleading was in my future. But when an injury changed my path I realized I could actuallylove a sport more than football and I found my place in competitive cheerleading at the age offifteen. I was no longer a football player but a football fan who coached cheer. Not a day goesby that I am not thankful to have found my calling in life at such a young age. This is what I doand I’m in it for life.

I have been coaching cheer for almost fifteen years now and bring with me all that I havelearned from countless experiences in the industry. I have had the privilege to work alongsidevery talented coaches, choreographers, gym owners and athletes and am grateful for all theyhave taught me. It has always been my dream to start my own competitive cheerleadingprogram and I’m so excited to be making this dream a reality.

I believe that every successful journey in life must start with a vision. My vision is to create agym environment where athletes can’t wait to get to practice, parents are excited for everyteam in the gym and a coach is not only a coach, but a mentor and a role model. I havesurrounded myself by a staff that shares my vision and is every bit as passionate about thissport as I am.

Every athlete deserves to feel that same passion and motivation every time they walk in thegym. I want them to know that they are part of something bigger than just a cheerleadingteam….it’s here that athletes will form bonds and friendships that will last a lifetime. It’s herethat athletes learn life lessons like discipline, teamwork and leadership that will shape them intosuccessful adults. And it’s here that athletes will gain the confidence and self respect to helpthem reach their full potential in life.

These positive results won’t come without a price; we expect our athletes to set goals andwork hard to achieve those goals, no matter how big or small they might be. Competitivecheerleading is just that - it’s competitive. While the focus can’t be all about winning, it’simportant that we train our athletes to succeed. So we will continually strive for athletes tocompete at their highest level and demand their very best 100% of the time.

We will help these athletes reach their full potential in cheer, but more importantly, help themto be better individuals. You will be part of a program that is committed to helping your childgrow into a good person, and we’re using cheerleading as our tool. All-StarRevolution will provide a family environment in which we can all take pride.

TRYOUT INFOThe tryout process is very brief and laid back. We want all cheerleaders to feel comfortable sowe do our very best to put them at ease as we get to know their goals for the upcomingseason. Each athlete will sign up for an individual evaluation time on one of our designatedtryout dates. Athletes should arrive 15 minutes prior to his/her scheduled time slot to warm-up and stretch. Although previous experience is not required, when the athlete’s name is calledhe/she may be asked to perform jumps, running tumbling, standing tumbling, stunts andflexibility. The tryout will conclude with a short interview. Each athlete should attend tryoutswith hair pulled up in a pony tail, wearing shorts, hot pants, lightweight tennis shoes and a t-shirt, sports bra or tank top.

To register and sign up for a tryout time, each athlete must:1) Turn in the 2011-2012 All-Star Registration Form included at the end of this packet.2) Turn in the Credit Card Authorization Form included at the end of this packet.3) Submit the non-refundable $55.00 registration fee and May 2011 tuition payment of $120.4) Request a tryout time slot upon registration or by emailing [email protected].

Tryouts will be held on the following dates:•Monday, April 18th 6:00 PM – 9:30 PM•Tuesday, April 19th 3:30 PM – 9:30 PM•Wednesday, April 20th 3:30 PM – 9:30 PM•Thursday, April 21st 3:30 PM – 9:30 PM•Saturday, April 23rd 11:00 AM – 3:00 PM•Monday, April 25th 6:00 PM – 9:30 PM•Tuesday, April 26th 3:30 PM – 9:30 PM

You can register at All-Star Revolution anytime between Monday, April 11th and Thursday,April 14th from 6 – 9 PM or during the scheduled tryout days. Walk-ins are welcome. You canalso register by submitting the required forms and payment via email [email protected].

Tryout Results:Team placement results (including practice day and time) will be posted to www.All-StarRevolution.com on Friday, April 29th by 5 PM. Results will also be posted on the All-StarRevolution gym window.

Required Parent Meeting and Grand Opening:There will be a required parent meeting for ASR athletes from 1 – 2PM on Saturday, May 7th at the All-Star Revolution Facility. We willanswer any questions and let you know exactly what can be expectedfor the 2011-2012 season. We will also have regular parent meetingsthroughout the season to establish effective communication with ourparents. Immediately following the meeting, there will be a grandopening from 2 – 4 PM. Athletes, family and friends are invited tocelebrate the beginning of ASR’s first successful season.

TEAM PLACEMENTAthletes are divided into teams based on two factors: age and skill level. Both factors are outlinedbelow to give you a better understanding of all-star cheerleading team placement.Age Breakdown: Show-Team Ages 3-5 as of August 31, 2011

Skill Level Breakdown:Athletes are evaluated based on their execution of tumbling, jumps, motion technique, stunting,and performance. All are important and taken into consideration when putting togethersuccessful teams. Preferred skills are listed at each level for your benefit.

Show TeamNo skill requirements

Level 1Tumbling-each athlete must have a cartwheel, round off and forward roll.Jumps-each athlete must demonstrate three different jumps.

Level 2Tumbling-each athlete must have a standing back handspring and a round off back handspring.Jumps-each athlete must demonstrate technical mastery of jumps (pointed toes, landing with feet together)including a double whip combination.Stunts-each athlete must demonstrate a level 2 stunt (examples: prep level liberty variations, extensions).Motions/Dance-each athlete must demonstrate an average understanding of motion technique.

Level 3Tumbling-each athlete must have a standing back handspring series and a round off back tuck.Jumps-each athlete must demonstrate technical mastery of jumps including a triple (whip) jump combination with atleast a level toe touch.Stunts-each athlete must demonstrate a level 3 stunt (examples: extended liberty variations, awesome full downcradle, inversion to sponge level, full up to prep).Motion/Dance-each athlete must demonstrate an above average understanding of motion technique.

Level 4Tumbling-each athlete must have a standing back tuck, standing back handspring back tuck, and either a runninglayout or a specialty pass through to a layout (example: front handspring, punch front, or whip through to layout).Jumps-each athlete must demonstrate technical mastery of jumps including a triple (whip) jump combination with anabove level toe touch.Stunts-each athlete must demonstrate a level 4 stunt (examples: extended liberty variations with a full down cradle,inversion to prep level stunts, full up to extended stunts, double down from 2 legged stunt, and kick single baskets).Motion/Dance-each athlete must demonstrate excellent understanding of motion technique.

Level 5Tumbling-each athlete must demonstrate a standing full, one to full or two to full/double and arunning specialty pass through to full or double full. Important-athletes without standingtumbling to full or specialty pass through to full, will be eligible for level 4.Jumps-each athlete must demonstrate technical mastery of jumps including a triple(whip) jump combination with back tuck, variety and above level toe touch.Stunts – each athlete must demonstrate a level 5 stunt (examples: extended libertyvariations including an overstretch with double down dismounts, full up to extendedliberty variation, and kick double baskets)Motion/Dance-each athlete must demonstrate superior understanding of motion technique.

ESTIMATED FINANCIAL OBLIGATIONSCompetitive all-star cheerleading requires a significant financial commitment. The following estimated costsmay be subject to change and other expenses are not foreseen, but could arise. To help familiescompensate with these expenses, All-Star Revolution has fund-raising opportunities available. Absolutelyno special financial arrangements will be made. Please carefully consider the financial commitment you aremaking.

•Registration $55 Due at registration•Monthly Tuition $120 Due monthly•USASF Membership $40 Due May 2011•Practice Wear $150 or less Due May 2011•Choreography Camp $185 Due June 2011•Uniform $250 or less Due July 2011•Competition Fees $850 Due August, September, October 2011•Accessories $100 or less Due November 2011

Registration:The $55 per athlete non-refundable registration fee is due up front to hold your tryout reservation. Noparticipants will be placed on an All-Star Revolution team or allowed to practice with an ASR teamwithout having paid this fee.

Monthly Tuition:Monthly tuition is $120 for all competitive team members. Families with multiple athletes will receive a50% discount off the additional siblings’ tuition. (All other fees will apply per athlete.) Tuition is due on the1st of each month and can be paid by check, cash or credit card. Any tuition fees not paid before the 6thof each month will be automatically charged to the card on file. Tuition fees not paid by the 10th of themonth will be charged a $20 late fee and $10 each day thereafter. Tuition fees and late fees are notrefundable for any reason.

USASF Membership:The United States All Star Federation (USASF) is currently the main governing presence in competitive all-star cheerleading. USASF is now requiring that ALL athletes certify to be eligible to compete during the2011-2012 season. All-Star Revolution will handle all the paperwork for the certification process. The feeis $40 ($35 for the membership and $5 for processing) and is due May 1, 2011. Any USASF membershipfees not paid by May 6, 2011 will be charged to the card on file. USASF fees not paid in full by May 10,2011 will be charged a $10 late fee.

Practice Wear:Athletes will be issued three full practice outfits. The fees for this apparel will not exceed $150 plus tax forfemale athletes (male athletes will be less.) The exact amount plus tax will be determined and posted toaccounts by May 1, 2011. Payment for practice wear must be received in full by May 6, 2011 or the cardon file will be charged. Any practice wear fees not paid by May 10, 2011 will be assessed a $50 late fee.

Choreography Camp:All athletes are required to attend the choreography camp for his/her team.The $185 mandatory choreography camp fee includes the cost ofchoreography, instruction and music production. All members MUST paythis non-refundable fee, regardless of attendance, which would also includemembers who join later in the season. The $185 choreography fee must be paidin full by June 6, 2011 or the card on file will be charged the balance. Any campfees not paid by June 10, 2011 will be charged a $50 late fee.

Uniform:Uniform fees will not exceed $250 plus tax. The exact amount plus tax will be determined and posted toaccounts by July 1, 2011. Mandatory uniform pieces include top and skirt for female athletes and top andpants for males. Additional items such as bows, warm-ups, backpacks, make-up, etc. are not included inthis uniform cost. Payment for mandatory uniform pieces (top and bottom) that is not received in full byJuly 6, 2011 will automatically be charged to the card on file. Any uniform fees not paid by July 10, 2011will be assessed a $50 late fee.

Competition Fees:Competition fees will not exceed a total of $850 per athlete for the entire season. Competition Feesinclude event registration fees for the season and an additional fee to cover staff expenses. The All-StarRevolution competition schedule for the 2011-2012 season will be determined based on several factors.Some of these factors include school squad conflicts, event location, the level of competition and a numberof other things. Once this schedule is set, the exact fee will be calculated and broken into three equalinstallments. The three equal payments for competition fees will be due by August 6, 2011, September6, 2011 and October 6, 2011. Any balances left on accounts will be charged to the card on file on the6th of each month. Each competition fee installment will be charged a late fee of $30 if it is not paid by the10th of each month in which it is due.*Please note that the above fees do not include travel and lodging for out of town events.

Accessories:Athletes may be required to purchase competition accessories such as make-up, hair bow(s), backpack,etc. The required items and exact cost plus tax will be determined and posted to accounts by November1, 2011. The accessories fee must be paid in full by November 6, 2011 or the card on file will be chargedthe balance. Any accessory fees not paid by November 10, 2011 will be charged a $25 late fee.

Any balances not paid by the 15th of each month may result in removal from the program.

ALL-STAR IN TRAININGWe understand that not everyone wants the time commitment or the financial responsibility that comeswith competitive all-star cheerleading, so we have included training classes that will enable all athletes tolearn the same skills as our competitive members. There will be a level 1 and level 2 class and each will beheld once a week for two hours per class. Training class members do not need to sign up for a tryouttime, but will be expected to follow the same registration procedures. Financial obligations:

SHOW TEAMFor athletes ages 5 and under we will offer a non-competitive show team. This team willpractice once a week for an hour and learn the same skills as a competitive team (on amore basic level.) Show team members do not need to sign up for a tryout time,but will need to follow the same registration procedures. Financial obligations:

TEAM COMMITMENTSCompetitions:Every athlete must not only attend every competition, but they must be available the entire

day/weekend - no exceptions.Upon arriving, every athlete must be in full uniform from head to toe. This means a clean

uniform top and bottom, hair out of face and secured in bow, full make-up and lashes, no showwhite socks, clean white shoes and no jewelry, nail polish, or gum.Remember that you are representing All-Star Revolution, your teammates, parents and staff.

Please represent ASR, and yourself, well.Athletes should have great sportsmanship, regardless of whether we win or lose. This includes

respect for athletes, parents, coaches AND toward other programs. We want to beperceived as a class act program so please act accordingly.We expect for every athlete to watch and cheer for other All-Star Revolution teams. This may

mean you may have to arrive earlier or leave later than required for your own performancesand/or awards. Poor sportsmanship by athletes or parents may cause dismissal from theprogram or other consequences.At competitions the main priority of a coach is to safely prepare the athletes for their

performances. After that, our next priority is to support our teams as they perform. Thisunfortunately means that, depending on the schedule, all of our team’s coaches may not be ableto stand with a teams as their awards are handed out, or, in rare cases (at events with multiplevenues) during a performance. We will do anything we can to get as many of the coaches there,but sometimes it is just not possible.Some competitions may require us to compete on a Friday and may require your athlete to

miss a part/full day of school. When considering competitions for our program, this is one ofthe first things we take into consideration. We will strive to keep our athletes from missing asmuch school as possible.It is important for our fans to be loud and supportive for ALL of our ASR teams. In fact, it

should sound like you are at a play-off game cheering on your favorite sports team.

Hotel Accommodations:We ask, but will not require, our athletes to stay in the “team hotel” for out of town

competitions. We will select “team hotels” when we release our competition schedule so thatthose that wish to stay in the “team hotel” can make reservations. This is a wonderful time forathletes and parents to get to know each other.Room blocks will not be made by All-Star Revolution so each family will be responsible for

their own lodging at out of town competitions.Teams may be required to meet at the “team hotel” any time throughout the weekend.

Choreography Camp:All teams will attend a 3-4 day camp in the ASR facility during the summer

to learn routines. This is mandatory to all athletes.Each camp will run from 9AM-4PM with an hour lunch break from

noon-1pm. Snack will be provided by ASR.For the safety of our athletes, they will not be allowed to leave the

premises. Please be sure to pack a nutritious lunch.Dates for each team’s camp will be released by May 16, 2011.

Practices:Each team will have a 2-3 hour practice once a week in the summer. During the school year

each team will keep that weekly practice and have an additional practice on Friday, Saturday orSunday (depending on the team.)Every athlete will be allowed to miss up to two practices during the summer.If for any reason there is a need to miss a practice (including the two summer absences) you

must submit the Absence Request Form to Edgar Ruiz at least 2 weeks in advance and it willeither be approved or denied via email. (See page 12 of packet for Absence Request Form)The greater the team’s attendance, the less extra practices will be needed.All-Star Revolution practice wear must be worn to all practices. You will get a schedule of what

outfit to wear at each practice.No jewelry can be worn in the gym at any time.Every athlete must be respectful to teammates, staff and parents at all times.We are here to work so please turn your cell phones off.There will be no food allowed in the gym area.You must arrive to practice on time. Five minutes early is better than five minutes late!If you are not working on a skill and you arrive early to practice, please stay off of equipment

and sit/stretch quietly and wait for your practice to start so as not to disturb other athletes inthe gym.If you are not an All-Star Revolution member or staff member, do not enter the gym area at

any time.Please try and stay healthy both in and out of the gym to help prevent any injuries from

occurring.

PROGRAM RULES AND REGULATIONS•I will not disrespect any staff, parents or athletes in any way.•Anyone threatening to quit or pull their child from a squad may be dismissed from theprogram immediately.

•I understand that athletes are required to wear the appropriate practice attire to everypractice.

•I understand the importance of punctuality and will arrive at all practices and scheduledevents on time.

•I will not participate in negative gossip in any way. This includes on-line communication.•I will strive to maintain and improve upon the skills I have and set goals to acquire moreskills.

•I will not keep my child from practice or performing as a form of punishment.•I will maintain good sportsmanship at all times when representing All-Star Revolution.•I will not use foul language.•I understand that at times an athlete will need to be moved from one team to another.•I understand that the parent viewing area should be a positive place, and I may beprohibited from using the parent viewing area if I cannot maintain a positive attitude orkeep comments to myself.

•I understand that uniforms are to be used only at competitions and should only be wornby All-Star Revolution athletes. I will take very good care of my uniform.

•I will not try to sell any All-Star Revolution items without the permission of Edgar Ruiz. Iunderstand that any violation to this agreement may result in legal action.

•I will not hold ASR responsible for any lost or stolen items since it is not possible tomonitor all valuables brought into the facility.

•I understand that if I plan to drop from the program, I must give withdrawal notice toEdgar Ruiz at [email protected] by the 15th of any month to avoid beingcharged for the next month’s tuition.

Medical Release and Policy AcknowledgmentI, the parent or legal guardian of the above named student hereafter referred to as “athlete”, do herebypermit the “athlete” to participate in gymnastics, tumbling, cheerleading or any other physical activitieswhile an athlete at All-Star Revolution, LLC hereafter referred to as “ASR”. By granting permission for“athlete” to participate in programs at “ASR”, I assume full responsibility for “athlete’s” personal safetyand release “ASR”, its supervisors and employees from any and all liabilities that may arise due to anyinjury including death to “athlete” by reason of “athlete’s” participation in any activity at “ASR” or in which“ASR” is participating elsewhere. I understand there is personal risk involved in any activity that involvesmotion, height or rotation and that these activities can result in serious injury, disability or death.

I authorize “ASR” to use photographs, video, and/or other likenesses of my child for use in its promotionalmaterials or sales and waive any rights of compensation or ownership thereto.

I understand all balances due on my account will be charged to my credit card on file on the 6th day ofeach month. If I do not want my credit card to be charged I must pay my account in full before the 6th dayof the month using cash or check. Any balance remaining on my account not paid before the 10th day ofthe month will automatically be charged a $20.00 late fee whether I received a statement from “ASR” ornot. I understand any payment for my account to “ASR” returned unpaid for any reason will incur a$25.00 bank fee plus a $20.00 late fee. I understand “ASR” does not refund tuition for ANY REASON.

I have read and understand pages 1 through 12 of the 2011-2012 All-Star Revolution Packetand agree to abide by its contents. I have kept a copy of this packet for my records.

CREDIT CARD AUTHORIZATION

All monthly balances due will be automatically charged to your credit card on file on the 6th dayof each month. If you do not want your credit card to be charged you must pay your account infull before the 6th day of the month using cash or check. All accounts must have a current creditcard authorization form on file. It is your responsibility to fill out a new CC authorization formfor lost/stolen cards or if your card expires. All applicable fees will be charged if the card on file isdeclined for any reason. After the 6th of each month, any discrepancies will be credited to youraccount for the next month’s billing cycle. Monthly fees not paid by the 10th of each month willbe charged applicable late fees. If your account remains unpaid past the 15th day of the month forany reason your child will be held out of practice and may be removed from the program.
