1. Welcome, introductions and apologies ActionSR welcomed all attendees, introductions given and the apologies were accepted.

This meeting was convened as a means of sharing intelligence and information between representatives of the London Borough of Newham and the commissioned providing service.

The minutes are a summary of discussions: NOT A VERBATIM RECORD.2. Minutes and matters arising from the last meeting

The minutes from the previous meeting were agreed and recorded to be accurate and the presentation was circulated.

SR went though the completed actions from the last Forums minutes held on 19/04/2017,

TAF part of todays agenda to present Adult Social Care Processes & Safeguarding

William Middleton (WM) notified to feedback on the Adult Social Survey 2017 at the October 2017 Forum

As Agnes Olagunju (AO) had been unable to attend today’s forum, therefore invited to the October forum to clarify medication terminology

Peter Turner (PT) invited to on behalf of CCG, sent apologies and therefore invited to the October 2017 Forum

TAF went through her presentation for Adults Social Care Process & Safeguarding.

TAF explained the criteria’s set under the Care Act listed for Adults Social being 18 or over, with either learning or physical disability and due to recent changes, includes carers.TAF stated the type of support they provide, i.e. Information, Advice and Guidance (IAG), where holistic assessments are carried out for each individuals to determine their needs and outcomes, this can also include volunteering services.TAF went through the breakdown of the teams the referral goes through within Adults Social Care with a diagram illustrating the initial contact to the last stage being monitoring and review.TAF further broke down the different team within Adults Social Care including;

Access To Social Care team, where initial referral are made, the Hospital Social Work team (based at Newham University Hospital) Access and Visiting Assessment Team responsible for the more urgent or

less complex needs Enablement team ‘Safeguarding Cluster’

SJ introduced himself as part of the Safeguarding Adults Team, Safeguarding Cluster, as managed by Debbie Allen.SJ informed the three ways in which providers, service users or family can make a referral;

1. A word format email to the Access inbox2. Raise a Safeguarding concern form on the Newham website, which gets

sent to the Adults Social Care inbox3. To call the Access team number on 0208 430 2000 and choose option 2

SJ explained it is necessary for providers to utilise option 2, by raising the concern form on Newham’s webpage SR urged the need of detailed completed forms clearly high-lighting concerns and needs.SJ high-lighted this due to the volume of referral being made and still finding these forms not filled out completely making it harder for the team to locate the missing information i.e. names and contacts, as every referral made makes it an obligation for the team to screen and Council to take responsibility.The Care Act has set three stage guidance to help assess risks;

Has the individual needs for care and support; and is experiencing, or is at risk of, abuse or neglect; and as a result of those care and support needs is unable to protect

themselves from the risk or experience of abuse or neglectAfter which, the team will make a decision to contact with the individual, referring person and family leading to the enquiry stage.SJ pointed out the sections 6 and 7 of the Care Act which bounds providers and the local government to collaborate and share informationSJ explained questions which need to be considered by providers before completing the concern form on behalf of a customers;

1. Has the customer got mental capacity around the concerns raised?2. Has the customer given consent?3. What is their desired outcome?4. What immediate action is required?

SJ & TAF however admitted the shortage of giving update to providers for their referrals and therefore acknowledges this and are trying to work on a better method.As pointed out by Provider, TAF & MO will consider the suggestion to add printing option when completing the safeguarding concern form, as a record can be kept for when completing on behalf of someone.TAF also suggested plans to perhaps set up a meeting with any provider individually where they have not received any feedback on concerns over 4 months old or to consider another inbox or area where providers can take their referrals as the system now puts a lot of pressure on Access Team. TAF has stated that issues have been identified and therefore will further look into solutions and feedback to SPF.

4. Workforce Development MSMS informed of the newsletter not being circulated during her leave however MS will continue the newsletter.

MS shared how the Provider Portal is going to be updated therefore Providers should check for updates.

MS urged the providers to sign up to the Access Care Knowledge data basis, which is the easiest way to keep up to date and would assure further subscription from Newham. Where providers are using this site, to maintain evidence of use. The providers attended where happy for MS to sign them on their behalf. Anyone who want to opt out/ or sign to send a request to MS, [email protected]

MS updated on the event held informing about Apprenticeship was a success and therefore are considering to hold another event.

5. Safeguarding Update MOMO introduced herself as the Senior Advisor Safeguarding. MO explained the difference between her team and the Safeguarding team discussed by SJ being how SJ’s team deal with individual referrals where as MO under take data collection, checking and identifying instances of multiple concerns.MO introduced Katie Burgess, the newest team member to assist with the volume of referrals they receive. Adults Safeguarding also oversees residents placed in Newham services from other Boroughs, this however will be transferred to Assessment & Care Management, where cases will go through a transferring process.MO noted how last year about 1,700 Safeguarding concerns forms have been raised although around 20% had been to report Pressure Ulcers where it may not have been preventable.MO presented the Safeguarding Adults Concern Form to the panel and pointed essentials to consider when filling it out;

The incident or accident to be high-lighted and making the concern clearer

To write the sections of the incidents/ accidents clearly, naming the location it took place.

MO noted the most referrals are received for financial and domestic concerns.

Action: IC to send out a copy of the Safeguarding Concern Form with the Minutes.

6. Brokerage Workshop NLNL presented the Brokerage Workshop PowerPoint Presentation.NL introduced the format of the Brokerage team as follows;

1 manager, being NL 7 Brokerage Officers 2 Contracts & Brokerage Assistance

NL explained what the Brokerage team does, which is commissioning services, as TAF stated, when her team completes assessments of residents, it is then given to Brokerage Team to commission the most suitable packages with those identified, services such as; home care, Community Access, Housing Related Support, direct payment and etc.Brokerage also produces and forwards the relevant paperwork to providers, liaising with customers, social workers and doing best to solve any problems.NL noted the following useful information;

Useful to know the customers CareFirst or Azeus (the new system to be in place) ID

To register with Egress, an encrypted, secure email as Brokerage is unable to send confidential information about their customers without sending it securely. If providers need to Brokerage does have a Egress Guide available which can be sent.

If needing to get in contact with Brokerage, to always use their group email and not only send individual officers, as to cover for emergency cases when officers maybe on leave.

Brokerage is a 7 day service, working over the weekend and on bank holidays.

NL provided the useful contact details; Brokerage phone number; 0203 373 0447 Brokerage group email address; [email protected]

The group then completed a quick workshop, where they were asked what they would like Brokerage to continue doing, stop doing and start doing?

A suggestion was made to continue calling up the care home to identify how many and what beds are available. NL explained how this is normally completed on a daily basis for the winter pressure months, however during other period it is completed more or less on a weekly basis, however NL can look into increasing these calls during summer time.

A suggestion was made to start replying or confirming recipient of emails, NL explained how this depends on the email itself and therefore not always possible. As Brokerage receives many emails for example of those informing of capacity, therefore it would be impossible to reply to all the emails and an automatic replies would similar to Access team may not be appropriate for Brokerage. For any emergency emails, NL stated to feel free to nag the officers otherwise can make her aware, but only when necessary as an influx of emails would make it harder for her to respond to these enquiries.

A suggestion was made to start providing the relevant paperwork for packages. NL noted the previous difficulties and informed of the new changes in systems means that Brokerage is unable to commission any new packages without social worker providing at least an interim support plan, unless an for an emergency package.
