I risultati del CABG oggi sono eccellenti!
• 3102 paz randomizzati a 1 o 2 mammarie
• Outcome primario: sopravvivenza a 10 anni (risultati a 5 y 2012)
• 67 chirurghi, 28 centri, 7 Paesi
• Mortalità a 30 giorni 1,2%, ad 1 anno 2,4%
• Stroke, IMA, re-rivascolarizzazione a 1 anno tutti < 2%
La curva di sopravvivenza è riportata
sovrapponibile a quella della popolazione di pari
età, persino negli ultraottantenni!

Nonostante i pazienti molto anziani che devono essere
sottoposti a CABG presentino con molte più copatologie
e la loro mortalità ospedaliera sia ovviamente più elevata,

proposed RCT of PCI versus CABG fully justifiable?
A. Bypass graft to mid coronary vessel offers:
•treatment of the culprit lesion of any complexity
•prophylaxis against future lesions in diffusely unstable endothelium
Redundancy
PCI/stenting only treats suitable localised proximal culprit lesions but
has no prophylactic benefit against new disease, which nullifies the
benefit of the stent
Single point of failure
It is arguable that PCI, POBA/BMS/DES 1/DES 2, will never match the
results of CABG

Conclusioni
ll numero di pazienti CCH > 80 aa è aumentato di 24 volte
negli ultimi 17 anni e destinato a raddoppiare
L’intervento può restituire una aspettativa di vita sovrapponibile a quella della popolazione generale
La chirurgia rimane il trattamento di scelta anche per gli ottuagenari
La tecnica più sicura è la rivascolarizzazione a cuore battente
L’ uso della doppia mammaria è sicuro e dovrebbe essere incrementato “no touch aorta technique” stroke
Necessari nuovi criteri di valutazione del rischio per l’anziano ed il paziente fragile in generale
