GENERAL INFORMATION

A parent or legal guardian applying to enrol a child in a government school should complete an Application for Enrolment form. Family details should include details of parents, guardians or carers residing at the same address as the student being enrolled. Only permanent residents of Australia and those children holding an approved visa sub-class number are eligible to be enrolled in a government school. Only students under the age of eighteen years of age can be enrolled and must be accompanied by a parent(s) or legal guardian.

It is a requirement of the Department of Education that any information on suspensions and exclusions must be provided to the school at the time of applying to enrol a child. This information will help the school to provide your child with the appropriate support, if required.

The Principal of the school may cancel the enrolment of an enrolled student if the Principal is satisfied that:

a) The enrolment was obtained by the giving of false or misleading information; orb) The Principal has received notification of changes to the following:

• Usual place of residence

• Court orders pertaining to the child

• Details of any conditions of the child that may call for special steps to be taken for the benefit or protection of the child or other persons in the school

• Legal guardian of the child

SECURITY AND CONFIDENTIALITY

The information provided on this form will be stored securely in local school and Departmental databases. The management of these databases is governed by State and Departmental policies to ensure security, privacy and confidentiality.

The Department of Education’s Information Communication Technologies Security Policy preclude this information from being used for any purpose other than to:

• determine whether your application for enrolment can be accepted;

• assist the school with addressing any needs for your child if enrolment is accepted; and;• comply with legal requirements or ministerial directions.

RESIDENT IN LOCAL INTAKE AREA

Duncraig Senior High School is a Local Intake Area School. The boundaries are determined by the Department of Education of Western Australia. A map of this area is available should you need to know these boundaries on the school website. An eligible child whose place of residence is within the local-intake area is guaranteed enrolment (subject to provision of required documentation).

Please note the following exception-The Gifted and Talented Selective Academic Program can accept enrolments from outside the Local Intake Area. Siblings of current Duncraig Senior High School students who reside outside of the Local Intake Area are not eligible for enrolment unless they are accepted into the Gifted and Talented Selective Academic Program.

Proof of Usual Residence in Area The school requires a copy of your current Council Rates Notice if owner occupied OR a copy of your Rental Agreement of 12 months’ tenancy, PLUS two recent documents with the same mailing address. These include: utilities account, government documents, Centrelink documents or drivers licence.

NOTE: Expiry date of lease agreements need to be updated before the commencement of the enrolling year.

LANGUAGE SELECTION All students at Duncraig Senior High School must study a language in Years 7, 8 and 9. The language the student chooses to study in Year 7 will be the language they continue to study in the coming years. Students do not necessarily have to continue with the language they studied at Primary School. Should your child wish to continue a language through to Year 12 the following should be considered: The School Curriculum and Standards Authority requires all students intending to enrol in a Western Australian Certificate of Education (WACE) language course (Yr 11 & 12) to complete an application for permission to enrol. The objective of the (Yr 11 & 12) language enrolment process is to ensure that students with a linguistic advantage in a language are not assessed against students who do not have the same advantage. Students deemed to have a greater linguistic competency than required by the course are not permitted to enrol in that course. For further information regarding the above, please do not hesitate to contact the Languages Department. DUNCRAIG SENIOR HIGH SCHOOL DRESS CODE The Department of Education Policy on school dress code allows schools to make the wearing of school uniform a requirement. The School Board has endorsed a dress code for all students attending this school. The School Board believes a school dress code:

PERMISSION TO PUBLISH STUDENTS IMAGES AND WORK FOR SCHOOL PURPOSES Your permission is sought for the school to publish video or photographic images of your child and/or samples of your child’s school work to be used by the school and the Department of Education. The purpose of using the images or work will be activities such as promoting the school, school events and student achievements. This memorandum relates to the connection and use of a student owned device at Duncraig Senior High School. This memorandum describes the terms of the provisions including level of service and scope of services agreed to by Duncraig SHS, the student and the student’s parent(s). Conditions The solution is supplied by Duncraig SHS to the student, based upon the following Agreement:

1. The student must take all reasonable care to prevent against any form of damage and loss of the student owned device.

2. The student will abide by all conditions outlined in the Students Online in Public Schools Policy.

3. The student and their parent will be solely responsible and legally accountable for any data stored or installed on the student owned device.

4. The student owned device and any software installed will be provided by the Parent and/or Student.

5. Student owned devices can only be connected to the school’s wireless network.

6. The Department strongly recommends that:

a. Student owned devices are installed with Anti-Virus protection which is either current or the version immediately prior to the current version.

b. Student owned devices are installed with the recent release of the anti-virus definitions files (one of the most recent four (4) released definitions).

c. Student owned devices have Operating System patches which are within seven (7) days of the vendor’s release date.

d. Student owned devices are enabled to receive auto-updates from the software vendor. A network account will be given to you by the school. In using this account, you agree to the following terms listed below. Our school provides access to Department of Education online services. These enhance the contemporary learning opportunities available to students and the range of teaching tools available to staff to deliver the Western Australian Curriculum. The Department’s online services currently provide students with access to: • individual email and calendar accounts; • the internet, with all reasonable care taken by central office and schools to monitor and control students’ access to websites while at school; • online teaching and learning services such as Connect, SEQTA, web-conferencing and digital resources; • online file storage and sharing services; and • these online services at locations other than school. I will only use online services for purposes which support my learning and educational research.

I understand that I am responsible for all activity in my online services account.

I will check with the teacher before sharing images or giving information about myself or anyone else when using online services.

I will keep my password private and not share with other students.

I will not let other people logon and/or use my online account.

I will tell the teacher if I think someone is using my online account.

I understand the school and the Department of Education can monitor my use of online services.

If I find any information that is inappropriate or makes me feel uncomfortable I will tell a teacher about it. Examples of inappropriate content include violent, racist, sexist, or pornographic material, or content that is offensive, intimidating or encourages dangerous or illegal activity.

I will not use the Department’s online services for personal gain or illegal activity (e.g. music file sharing), to bully, offend or intimidate others or access or send inappropriate materials including software that may damage computers, data or networks.

I will acknowledge the creator or author of any material used in my research for school work by using appropriate referencing.

I will get permission from the copyright owner of any material used in my school work before I reuse it in a portfolio for employment, in a competition or any other uses other than for my private research and study.

I will use appropriate language in all internet communications.

I will not try to access internet sites that have been blocked by the school or the Department of Education.

I will not damage or disable the computers, computer systems or computer networks of the school, the Department of Education or any other organisation.

I understand that:

I will be held responsible for my actions while using online services and for any breaches caused by allowing any other person to use my online services account;

the misuse of online services may result in disciplinary action, determined by the principal in accordance with the Department’s Behaviour Management in Schools policy; and

I may be held liable for offences committed using online services. A breach of any of these conditions will result in suspension of your network account and possible further consequences. PERMISSION FOR STUDENTS TO HAVE AN ONLINE SERVICES ACCOUNT

I give permission for my child to have an online services account. I understand and agree that my child has responsibilities when using the online services provided at school for educational purposes, in accordance with the Acceptable Use Agreement for school students. I also understand that if my child breaks any of the rules in the agreement that the principal may take disciplinary action in accordance with the Department’s Student Behaviour Policy and Procedures. Note: While every reasonable effort is made by schools and the Department of Education to prevent student exposure to inappropriate online content when using the Department’s Online Services, it is not possible to completely eliminate the risk of such exposure. The Department cannot filter internet content accessed by your child from home or from other locations away from the school.
