    Evidence of any kind.Damage, equipment involved, position of

    equipment and tools etc.

    Position of any injured person prior toand AFTER the accident

    Spillages, stains, clothing.

    Check on any injured person (s) but

    before you do make sure that the doctor

    confirms he/she is fit to receive you.

    Try and get information from them but

    do remember they are HURT.

    Make sure you have awitness when you speak

    to them, preferably medical.

    Get a statement fromthem if possible and get

    them to sign it, again ifpossible.

    After talking to the

    injured person now talkto the doctor, his/herinformation will be veryvaluable.

    ACCIDENT/INCIDENT INVESTIGATIONNOW YOU CAN TALK TO ANY OTHERWITNESSES.

    Although we must never apportion blame

    It is certain that something or someonecaused the accident. Witnesses may

    well be frightened and/or guilty.

    You must be able to put witnesses at ease

    but at the same time get all you can from

    them. EVEN IF IT IS LIES!!!!!!!

    Try and segregate witnesses but do itsubtly to avoid fear. If you do not segregatethem, they can swap stories and establisha common approach.

    Assure them of your intentions i.e. ONLY to

    establish cause.Ask them if you can take notes.

    Let them see what you are writing

    Do not take notes while theyare talking.
