Two groups of dental students, with no experience in operative dentistry, were trained solely by either VR or conventional training in the pre- paration of conventional class 1 cavities. The subjects all used the same operative armamentarium and phantom heads, and were allocated the same duration of practice periods. At the comple- tion of these training periods, both groups produced two class 1 cavities on the lower left ®rst molar, which were subsequently coded and blindly scored for the traditional assessment criteria of outline form, retention form, smoothness, cavity depth and cavity margin angulation. An ordinal score of 0±3 or 0±4 was assigned for each assessment criterion: the higher the score, the worse the evaluation. After initial independent scoring, the two examiners discussed any notable differences until an agreed score was reached. Once the codes were broken, non-parametric analyses were performed on the data. Wilcoxon Tests for the semiquanti- tative scores indicated signi®cant differences between the VR and conventional training groups for outline form, depth and smoothness but not for retention or cavity margin angulation at P < 0.05 level, with the VR group receiving the higher, i.e. worse, scores. Cavity margin angulation approached signi®cance with a P-value of 0.0536. The results indicated that VR-based skills acquisition is unsuitable for use as the sole method of feedback and evaluation for novice students. The current VR-based training unit is used in undergraduate pro- grammes in three continents. Despite the rapid intro- duction of this new technology over the last 5 years, little objective research has been published to substanti- ate its purported bene®ts. In professional training, increasing emphasis is being placed on standardization of teaching protocols and objective evaluation. With respect to skills acquisition, VR-based training has been utilized for the repeated use of a standardized simulated patient (1, 2), with standardized treatment scenarios. The dental `patient' simulator used in this study has been described elsewhere (2, 3). The system's software allows for the representation and evaluation of a num- ber of operative and ®xed prosthodontic procedures. It is postulated that the use of these simulations would be superior to conventional training, with plastic teeth, as the individual layers of the natural tooth are repre- sented in the virtual tooth, including dentinal carious lesions (Fig. 1). The virtual representation allows preparations to be viewed from many angles and at varying, operator controlled, magni®cation. This may improve students' understanding and self-critical evaluation of cavity design and preparation (4±6). The image of the virtual mouth and tooth also provide real-time information feedback of the actual preparation cut on the tooth analogue. Individual sessions may be recorded and replayed at will, providing positive feedback and encourag- ing improvement in technique, through individual practice (7).
