Data from 2000/01 school year found that approximately 14% of primary and 12% of secondary school students were obese. 1 It was higher than the figures reported in last decade. 2, 3 Obesity is a well-known major risk factor of cardiovascular and metabolic disorders. However, the impact of obesity on sleep in children is frequently ignored in the literature. Sleep problems can lead to deterioration of day-time achievement and social life. This paper will discuss the impact of obesity on sleep as well as the management of obesity in children. Obesity in Children Commonly used definition of obesity included weight greater than 120% median weight for height (MWH). 4, 5 Local reference data were available. 6, 7 BMI (Body mass index) is less useful in paediatric population as adult cut-off value of 30 kg/m 2 for obesity and 25 kg/m 2 for overweight could not be directly used in paediatric population. It is because normal BMI is dependent on age and gender. International cut- off values of BMI for defining obesity and overweight at different age and gender were reported. 8 Thus, a statistical norm should be used in pediatrics population with respect to age and gender. For Hong Kong, the cut-off values for obesity is 97 th centile in boys and 98 th centile in girls and the cut-off values for overweight is 88 th for boys and 90 th for girls. 9 For practical purpose, it is convenient to use the cut off of 90 th centile and 97 th centile of BMI as the definition of overweight and obesity in Hong Kong children. Local data on BMI distribution were available. 9 General practitioners should routinely measure both weight and height for children attending their clinics. These data should be charted and track changes overtime. Weight reduction or maintenance regime should be recommended if a trend towards obesity was found. Early identification and correction of obesity in children should be the key to prevent adverse outcome of childhood obesity. IntroductionPrevalence of obesity is increasing in Hong Kong. Datafrom 2000/01 school year found that approximately 14%of primary and 12% of secondary school students wereobese.1 It was higher than the figures reported in lastdecade.2, 3 Obesity is a well-known major risk factor ofcardiovascular and metabolic disorders. However, theimpact of obesity on sleep in children is frequently ignoredin the literature. Sleep problems can lead to deteriorationof day-time achievement and social life. This paper willdiscuss the impact of obesity on sleep as well as themanagement of obesity in children.

Obesity in ChildrenCommonly used definition of obesity included weightgreater than 120% median weight for height (MWH).4, 5

OSAS as compared with normal control.10 A review ofcases referred to our sleep laboratory for polysomnographyfound that nearly half of obese children referred to ourlaboratory had OSAS.11 The major underlying reason islikely related to the adipose tissue deposition in pharynxcausing obstructions in the airway while asleep.

Obese children with OSAS face a double challenge as bothOSAS and obesity imposed risks to the cardiovascular system.9

Children with OSAS were found to have impaired daytimeperformance,12-14 hyperactivity15 and failure to thrive.15

As the co-morbidity of OSAS and obesity impaired childhealth and daytime performance of children, OSAS in obesechildren should be promptly diagnosed.

Management of OSAS in Obese ChildrenGeneral practitioners are the first line medical personnel whocan screen obese children for OSAS. General practitionershould actively ask for habitual snoring, the hallmarksymptom of OSAS in obese subjects at consultation. Blundenet al found that only 15.2% of parents of children with highrisk of sleep problem raised sleep problems to GP.16 For obesechildren with habitual snoring and other complaints likeexcessive daytime drowsiness or fatigue, unrefreshed sleep,impairment of daytime performance and cardiovascularabnormality, e.g. hypertension, OSAS should be suspectedand referral to overnight polysomnography should be made.

Adenotonsillectomy (T & A) should be considered inmoderate and severe OSAS. Weight reduction should beencouraged in obese children with co-existing OSAS as Williet al17 reported resolution of sleep apnoea after weight lossin 5 morbidly obese children. As children with OSAS hadincreased energy expenditure during sleep (SEE),18 one shouldbe aware that treatment of OSAS by T&A may be associatedwith weight gain secondary to decreased energy expenditureduring sleep.18 Close follow-up after resolution of OSAS isessential to detect and manage early trend of further weightgain which may lead to relapse of OSAS. In obese childrenwho has persistent OSAS after T & A or those who are notfit for surgical treatment, non-invasive ventilation can beoffered together with weight reduction.

Weight Reduction RegimeA weight reduction program requires a carefully plannedcalorie-reduced diet and a regular exercise program. A studyin pre-adolescents found no weight loss if only dietaryrecommendations were given without exercise.19 Thereshould be close monitoring by physicians to avoidcomplications. Input from psychologists to providebehavioural therapy to patients and parents are helpful inimproving the outcome.19-21

Dietary Intervention

For older children and adolescents, a weight reduction dietusually provides 1,000 to 1,500 kcal daily (4.2 to 6.3 MJ).Diet less than 1,000 kcal is usually not advised.22 The dietshould provide at least 60% of estimated averagerequirement for the child’s age.22 The weight loss is usuallymore marked in first few weeks and less subsequently. Arealistic and practical target should be set. Half to onekilogram weight loss per month is usually well tolerated. Inan active growing child, maintaining a static body weight isalso an acceptable alternative. A practical and achievablegoal should be set at the start to increase chance of successand prevent abandonment.

Exercise Programme

Moderate exercise is usually recommended for weightreduction. Different exercise levels are defined by heart rate.23

Intensity of exercise level is commonly classified by metabolicenergy equivalent (MET) to facilitate study in the field withoutheart rate monitor. One MET refers to activity equivalent toeating or relaxed standing or supine rest. In the authors’department, children and their parents are asked to notedown the exercise level associated with the exercise programdesigned by the physiotherapist. (Table 1) They are asked toundertake similar moderate exercise level, as measured bysubjective assessment of exertion level, with other personallypreferred activities 5 times per week. Exercise interventionalso served as a life style modification in obese children tobreak their sedentary life style.

Life Style ModificationGenesis of childhood obesity depends on both non-modifiable factors and modifiable factors. Non-modifiablefactors included genetic factor, gestation age and ethnicity.However, the actual causes of obesity depended heavily onmodifiable factors. One of the most important modifiablefactors is the perception of obesity by children and parents.From authors’ personal observation, it is common for obesechildren and their parents to neglect the health consequencesof obesity and accept their current status. Unawareness ofthis problem and continuation of their unhealthy lifestylemay further increase the degree of obesity. Family physiciansshould identify such a problem and discuss with both theobese children and parents for the adverse consequences ofobesity. Effective interventions should be recommended toobese children. In addition, education to both the childrenand parents on healthy lifestyle is very important. Anappropriate dining pattern should be recommended. Healthyeating pattern involving the input from children, parents andschools. Family meals should always be recommended as itensured a more nutritious meal and good eating habits.24 Arecent studies show that more family mealtime per week isassociated with better health and psychosocial well beingof adolescents 25 Parent could serve as a role model of healthyeating habit for children during family mealtime.

Parents should avoid using food as a reward. Sedentarypursuits should be limited, for example excessive televisionviewing, video gaming and Internet surfing especially thoseassociated with snack food and fizzy drinks consumption.Parents should encourage an increase in casual activities,e.g. walking to school, and reduced usage of remote control.In addition, long-term engagement of sport should berecommended for obese children.

ConclusionChildren with obesity and OSAS face a vicious cycle of obesityworsening OSAS which in turn leads to increasedconsumption of food to ward off daytime sleepiness.Together, these two phenomena have an exaggerated impacton different systems, especially the cardiovascular system.GP should be prepared to work with the family to break thevicious cycle and make referral for sleep studies in thoseobese children with snoring.

A 30 year old gentleman, who enjoyed good past health,complained of recent fast palpitations that were abruptin onset. One day, he visited the ER department withpalpitations that lasted for more than 1 hour. Onadmission, the blood pressure was 120/70 mmHg. ECGwas done and was shown below.

What is your diagnosis?

a. SVT with aberrancy

b. Atrial fibrillation with aberrancy (AF)

c. Ventricular fibrillation (VF)

1. Which of the following(s) is/are correct (you may choosemore then one item)?

Options d and f are correct. This gentleman was in sinusrhythm with normal axis. The P wave amplitude increasedto 4 mV and was peaked in shape. By definition, any Pwave amplitude greater than 2.5 mV would be classifiedas P pulmonale, which signified right atrial enlargement.There was no ventricular hypertrophy. There were frequentatrial ectopics with slightly variable morphologies of Pwaves.

2. After admission to the ward, he complained ofpalpitations again, ECG was done and was shown below.Blood pressure was 130/80. What was the arrhythmia?

Option C is correct – patient had atrial flutter with 2:1 block.The second ECG showed a ventricular rate of around 170/min. The atrial rate was around 300/min and was saw-toothed especially in the inferior leads.

Atrial arrhythmia is common in patients with COAD. It isusually difficult to convert it back to sinus rhythm duringthe acute exacerbation. The aim is to control the heart rateand to relieve hypoxemia so that it will convert back tosinus rhythm afterwards spontaneously. Indeed, the heartrate of this gentleman was initially controlled with diltiazemand it reverted back to sinus rhythm spontaneuously afterthe exacerbation of COAD subsided.

A 55 year old Chinese male noticed periungual swellingover his left index finger for over one year. The lesion grewin size slowly and was occasionally tender. There was alsodeformity over that finger nail. There was no precedingtrauma. His general health was good. Physical examinationrevealed a 5 mm translucent cyst with depression grooveover the nail.

Dermatology Series皮膚科病例研究

A 28 year old Chinese male noticeddarkening of the skin over his axillaefor few years. There was nosymptom. His general health wasgood. Physical examination revealeddarkening of colour, roughness,increased skin markings and drynessover his axillae and back of neck.Multiple skin tags were also present.His body weight was about 85 Kg.

Answers to April 2005Dermatology Series皮膚科病例研究二零零五年四月份答案

1. What is the clinical diagnosis?The diagnosis is acanthosis nigricans (AN). This patient hasthe pseudo-AN subtype.

2. What are the different subtypes of this dermatosis?There are five types of AN. Type 1 is the hereditary, benignAN. Type 2 is the benign AN and is associated with variousendocr ine d i sorders such as type 1 d iabetes ,hyperandrogenetic states, Cushing’s disease, Addison’sdisease and hypothyroidism. Type 3 is the pseudo-AN andis associated with obesity. Type 4 is the drug-induced ANand may be due to nicotinic acid, stilbestrol or oralcontraceptives. Type 5 is the malignant AN and usually dueto adenocarcinoma, or lymphoma. In type 5, ANinvolvement is more extensive and severe; and can involvelips, palms and soles.

3. What is the underlying mechanism?AN is caused by hyperinsulinemia associated with insulinresistance. Insulin binds to insulin-like growth factorreceptors in the skin cells, resulting in hyperplasia of skin.

4. What are the investigations?There is elevated fasting blood insulin level. Screening fordiabetes should also be done. Further evaluation to ruleout endocrinopathy and malignancy are usually notrequired in obese patients or those with l imitedinvolvement.

5. What are the treatments?Weight loss can resolve type-3 AN and has beneficial effecton other types of AN. Improvement may be obtained withtopical keratolytics, tretinoin, corticosteroids, dermabrasion,laser and cryotherapy. More severe form of AN may betreated with isotretinoin or acitretin. It should be noted thatpatients with AN are at a higher risk of atherosclerosis andsubsequent cardiovascular disease, hypertension,dyslipidaemia and impaired glucose tolerance.
