2016 House Staff Orientation 1
Welcome!
Please note:
• Attendance is being monitored. To receive your orientation bonus, you must participate in all Orientation activities (except the Welcome Picnic or anything noted in this schedule as applying only to certain house staff). At Registration the first day, you will get a personalized attendance badge. Don’t lose your attendance badge and don’t forget to bring it with you each day! It is considered unprofessional behavior to leave a session in progress.
• Remember to Bring to Orientation the following items: o Particulate Respirator Approval Form (in packet mailed to you or here:
https://www4.vanderbilt.edu/safety/respirator/pdf/appendix_3_n95_initial_fit-testing_form.pdf).
o New Employee Information Form (in packet or here: http://hr.vanderbilt.edu/forms/documents/NewEmployeeInfoForm.pdf).
o W-4 (in packet or here: https://hr.mc.vanderbilt.edu/forms/2016W4.pdf). o Identification documents (original and unexpired) for completing your I-9
(https://hr.mc.vanderbilt.edu/i9/documents/I9-Acceptable-Documents.pdf). o Parking Agreement (in packet or here:
http://www.mc.vanderbilt.edu/documents/gme/files/PARKINGagreement.pdf). o State-issued Vehicle Registration (any state) o If your program rotates to the VA, two official IDs for VA Registration (i.e. Driver’s License and
Passport - other options listed here: http://www.mc.vanderbilt.edu/documents/gme/files/PIV%20IDENTIFICATION%20MATRIX.pdf)
• Vanderbilt photo ID card: You must have your official VUMC ID card before you begin your training program. This ID includes library access, meal card, and building access systems. The Card Services Office is located in D-2107 Medical Center North (MCN). (As noted above, it is considered unprofessional behavior to leave a session in progress – even to get your ID badge made – and attendance is being monitored.)
Card Services Office Hours: Monday ǀ 8:30am-10:15am and 2:00pm-4:30pm Tuesday through Friday ǀ 8:30am-1:00pm and 2:00pm-4:30pm
• Orientation attire is business casual, but be sure to wear comfortable shoes.
• Male house staff should be clean shaven (morning of or right before) for respirator fitting.
• Nursing mothers: A Staff Lactation room which contains three private booths is in room 10600DA Critical Care Tower. The Critical Care Tower is in the main hospital. From Light Hall, go across the courtyard to the hospital elevators. Follow the signs toward the cafeteria, and you will come to the CCT elevators. Take the elevator to the 10th floor. The lactation room is near the CCT 10th floor elevator lobby, near the stairwell.
• Wi-Fi: You can connect to the network ‘vummiv’ with no security settings. This is a public Wi-Fi connection so use care when transmitting sensitive data. After your start date, you will be able to connect to the secured Wi-Fi network ‘vuNet’. If you have any issues connecting to it, the VUMC help desk (615-343-HELP) or your program’s IT department can help you connect to it.
• PCP: To find a primary care physician at Vanderbilt, call 615-936-MYMD.
