T H I S Y E A R ’ S

HIGHLIGHTSIt is Autumn, a season of change. Everywhere I

go there is change. It seems the predominant

topics at all the meetings I attend deal with

change. Well, you know, "Change" has been the

mantra of our current President, during the

political campaign and now. Unfortunately, I

think some are beginning to view change as a

four letter word.

Change can be a lot of things, but it always

seems to be disruptive. It makes us evaluate the

habits, good and bad, that we have maintained.

It forces us to re-evaluate and prioritize. It helps

to clearly focus on what we value, for an

organization our mission.

Genesis World Mission is in a season of change.

Like the change in the weather initiates the

change in the foliage, the changes in our health

care environment along with a desire to improve

the quality and efficiency of our patient care have

necessitated our changes. Many of the changes right now may be subtle and not readily

noticed, like a warm fall brings about a more muted change in the trees. However, eventually

there will be a more dramatic change. With change we often face tough decisions, the lose

of the established, familiar, and comfortable. Hopefully the net gain is positive, valuable,

and worth the effort.

As we have had many in depth, soul searching, and sometimes contentious discussions

over the past year here at Genesis World Mission, we are determined that some things will

not change:

Our mission: "To convey the love of Jesus Christ by meeting the physical,

medical, and spiritual needs of the less fortunate, both globally and locally."

Our value of Faith, People, Partnerships, Community and Giving.

Psalm 46 reads "God is our refuge and strength, an ever present help in trouble. Therefore

we will not fear, though the earth should CHANGE."

I am looking forward to seeing what success the changes of this season will bring!

We know a “house” doesn’t make a “home,” but the infrastructure

of a roof over your head is pretty important. Building a home takes

a team of contractors: electricians, plumbers, framers, painters, and

somebody to direct them all – a general contractor.

Similarly, establishing a medical home requires the coordination of

many team players, but this is not how our current medical models

of care work. The US truly has no medical “system.” It is a

complicated web of competing industry practices where patients

often wander around confused, creating a long trail of scattered

records. Most of the time patients do not receive focused, consistent

advice on overall prevention and education on health issues – current

reimbursement models just do not pay for that. They have no central

home which knows and understands them fully, advocates for them

and helps them move to higher levels of responsibility and

self-sufficiency. Patients are seen as numbers, statistics, diseases and

billable hours. Rarely are they fully-recognized as interacting human

beings with interacting medical, social and spiritual needs.

In the medical home model, the family physician is the general

contractor, who has the ability to pull disparate disciplines together

to meet the patient’s variety of needs. He or she utilizes every team

member’s highest skills so they are all practicing at the top of their

training. These team members include general and specialist

physicians, mid-level providers like NP’s, PA’s and hygienists, as

well as social workers, RN’s, counselors, and even lay people to

provide a team of continuous, holistic care for the patient. We should

not forget that probably the most important member of the team is

the patient himself – his body is the home he has to live in, after all.

Overseas, a medical home may follow more of a village model,

simply because the basics of sanitation, access to clean water, and

rudimentary health knowledge are as important as direct medical

care. In order for these systems to come together, it truly does “take

a whole village” for the ongoing health of any one individual to be

complete.

BUILDING A MEDICAL HOME

A Medical Home is a team of professionals looking after

the patient’s best interest.

In many ways the Garden City Community Clinic has functioned

under this model since its inception, blending medical, dental,

mental health and social work under one roof.

But to take it to the next level, we depend on the community to help

us. Our next steps are to implement electronic medical records so

every team member has access to the entire picture of the patient’s

health. We will also need to hire medical professionals such as RN’s

and medical providers to allow for greater continuity of care.

During the past year, across all of its activities, Genesis World

Mission has continued to develop and implement the means of

bringing together disparate resources to serve those who are in

desperate need. In the year ahead, with the concerted focus, energy

and dedication of our board, staff, volunteers and donors, we intend

to keep ‘building’ so that each of our patients—present and

future—will know the strength, security and compassion of true

medical homes.

Jeanie* had worked in the service industry all her life and had long dealt with chronic hernias and had multiple

surgeries with limited success. When her husband’s leg was amputated following an accident, their medical

insurance dropped both of them.

After hearing about the Garden City Community Clinic, Jeanie came to us for treatment for depression, high

blood pressure, high cholesterol and tooth pain. Through the Clinic and the Volunteer Physicians Network (VPN),

Jeanie was able to get imaging, a gastrointestinal consultation and a colonoscopy, with services provided by

Intermountain Medical Imaging, Saint Alphonsus and Dr. Nic Cordum at the Digestive Health Clinic.

“I was called personally by a doctor, Jeanne marveled. “ I have never been called by a doctor before!”

Now Jeanie is getting medication through the Garden City Community Clinic’s Genesis Assistance Program

(GAPmed) program, and she recieved dental services.

Though Jeanie still struggles with pain, she is now hoping to return to work, to manage her family and her

household, and to continue enjoying her grandchild.

“I appreciate the help more than you will ever know. I want to say so much more than thank you, but I don’t know

how. I wouldn’t be here without you.”

With a history of struggles, Jeanie is a survivor. Though her story is unique, she represents the struggles that our

patients deal with on a daily basis. We are here for them!

Community Collaboration

The partnership between the

Family Medicine Residency of

Idaho and the Garden City

Community Clinic remains strong.

This past year, FRMI has arranged

to utilize the Clinic space for four

1/2 day time slots when it would

otherwise sit idle. Currently,

refugee populations are being

immunizations, and behavioral

health screenings and OB patients

of Dr. Sarah Cox are being seen in a

location more convenient than

FMRI’s Fort Street clinic.

The Garden City Community Clinic provides charity medical, dental and mental health care to those in the

Treasure Valley who are economically disadvantaged and without health insurance. Volunteer health professionals

make most of this care possible. All care provided is free of charge to eligible individuals.

Located in the heart of Garden City, which is a federally-designated Medically Underserved Area of Boise, the

Clinic provides medical services on three days each week and on-site basic dental services twice weekly. The

dental work has grown in its capacity and is now also offering oral hygiene services as volunteers are available.

Mental health care is a massively underserved need in Boise and we do our part. Unfortunately in the last half of

this year we have faced the same challenges as the rest of the community: a lack of mental health providers, in

our case volunteers. Because of this, we have had to scale back our mental health services, but social work,

counseling and patient education are still offered alongside medical services.

Area hospitals and medical groups donate laboratory and radiology services for our patients. Many of the medical

supplies necessary for normal clinic operations are donated and other items are purchased at reduced cost.

Want to help? If you are a health professional and would like to discuss providing volunteer medical, dental or

mental health assistance at the Clinic, please contact our volunteer coordinator, Kristal Smith,

opportunities are also available.

Genesis Assisted Pharmaceuticals One essential supportive service is providing prescription medications for our patients who,

without insurance, face impossible costs. For direct dispensing, the Clinic continues to maintain a

medication sample “closet” stocked with donated and purchased pharmaceuticals. The Genesis

Assistance Program (GAPMed) applies directly to drug manufacturers on behalf of patients,

making it easier for those clients to receive essential pharmaceuticals at no charge.

As part of our initiative to increase patient’s self-responsibility, we began to require some to

purchase their generic prescriptions if they were on a “$4 formulary list” at local pharmacies. This

fall, we are beginning to move all generic prescriptions out to Ladd Pharmacy on Broadway at a

reduced cost. For those who cannot afford even that, the Clinic will still pay for the drug at cost,

allowing our staff to be freed from inventory control work. If necessary, complimentary

transportation to Ladd’s can be arranged for patients.

During the 2009-2010 fiscal year, over 236 volunteers provided more than 6700 volunteer hours. This represents at least $132,247 worth of donated services on behalf of the patients we serve. In addition, more than $123,000 worth of professional services were donated including radiology, lab/pathology, marketing, accounting and other consulting services. Thank you to all the healing people who are part of this community.

The Builders Continued...

Volunteers of the Year

During the past three years, two volunteers have served almost every week to o�er spiritual care to

the patients of Garden City Community Clinic. Geri Williams and Rhonda Willoughby paired up in 2007

as a prayer ministry team and are found welcoming patients during clinic hours in the waiting room.

With hugs, smiles, and prayers, Geri and Rhonda touch people at their deepest places of need. Not

only have they supplemented the medical care received at the clinic, they have also ministered to

volunteers and sta� through their own joys and challenges. Together the two have logged over 220

clinic evenings of volunteering, representing 307 hours in this position. In addition, Rhonda served as

our AP bookkeeper for about a year, logging another 62 hours of volunteer work there. Congratulations

to Geri and Rhonda as the Genesis World Mission's 2009 Volunteers of the Year.

Connie had been seeing a doctor for about five years for pain and numbness in her back and arm. The doctor told her the problems simply were in her head and that she was fine. But her condition continued to worsen, and eventually Connie could hardly walk out of the doctor’s office. She ended up in a motorized wheel chair. She could no longer use her left side, and she was beginning to have numbness in her right arm.

When she could no longer work, Connie lost her medical insurance. She found her way to the Family Medicine Health Center, managed by Idaho’s family practice residency program. Dr. Brandon Isaacs immediately recognized the problem. He referred her through VPN to a specialist, Dr. Ronald Jutzy, who Connie describes as a “neurosurgeon extraordinaire!”

Dr. Jutzy performed a complex surgery, and the results have been remarkable. The numbness in Connie’s right arm has gone, she has use of her left arm and side, and most of the left arm numbness has subsided.

Connie says she is almost back to 100%. She can walk once more, take care of herself, and play with her new grandchild.

The Volunteer Physician’s Network

is win-win-win for clients, doctors

and the community clinics.

The Volunteer Physicians Network (VPN) serves to simplify the process of connecting volunteer specialist physicians with patients in need of charity care who have a medical home. Genesis World Mission manages this four year old network, allowing four Treasure Valley clinics access to hundreds of area specialists, imaging facilities and lab who donate their services.

Our goal is to recruit all doctors within each specialty area to be involved in this program. This spreads out the generous donation of specialists’ time, energy and skills so that no one specialist is called upon too frequently. Currently, VPN has 237 participating specialists and in four years since its inception, has facilitated more than $3.7 million in donated health care services.

After 18 months of work, VPN began serving Canyon County in June 2010, opening up access to Terry Reilly Health Services there, as well as the newly formed Canyon County Community (free) Clinic which opened in January. More and more underserved people in Canyon County will now be able to be referred to specialists and receiving the additional health care they need. The number of Canyon County specialists is steadily growing.

“It’s not just about health care,” says VPN Manager Barbara Mahr. “Specialist interventions can lead to health improvements which stop the chain of poverty, allowing clients to resume happier and more productive lives.”

A SPECIAL REPORTA Rapidly Changing Health Care Environment

When President Obama signed the most sweeping health care legislation since Medicare, the

“two minute clock” started ticking for the Garden City Community Clinic. With a mandate that

all Americans will have to carry health insurance by 2014, and a huge expansion in the federally

funded Community Health Center program, the changing environment poses some serious

questions about our future:

everyone will be able to afford medical care?

initiatives have on the generous support which now sustains our operations?

Here’s the crux of it from our point of view: As the Federal health mandates begin to move

fully into place by 2014, we think the public support of free health clinics is going to subside.

We suspect the tide may turn against those who are getting a ‘free ride.’ People may say:

“Everyone should have insurance now, right?” “Everyone’s protected. Why do we need charity

clinics?” And it seems likely that if donors hold such perceptions, they may diminish or

withdraw their vital support.

So we know we will need to develop ways to respectfully address these perspectives. We believe

that no matter what the government does, there will always be many people who cannot afford

health care, be it basic coverage or out-of-pocket costs. There will always be those who need

pharmaceutical assistance. There will still be those who need dental care, which, as it happens,

is not covered in the national plan. There will definitely be those who need help navigating the

increasingly-complex health system, especially the newly covered pool of 30 million people.

First, we must continue to provide health care services for those who cannot afford insurance or

health care in ways which improve access and which are based on a holistic, patient-centered

approach. This summer, we examined the possibility of the Garden City Community Clinic

applying to become a Fully Qualified Health Center. There are 350 slots opening up across the

nation for “New Access Points” this fall and billions of dollars available for caring for the

underserved. In the end, we decided not to pursue this opportunity for a variety of reasons.

Second, we must help as many of our clients as possible to learn to balance personal

responsibility with community responsibility and to progress beyond the status quo to a place

where they no longer need our services. We have already begun to do this with our

pharmaceutical programs. We continue to explore community-based clinic models that empower

patients to make healthier decisions. This is a systematic issue that needs the input of the entire

community, both those who provide services and those being served. In the end, we believe

sustainable and accessible health care is a national problem with localized solutions.

How the clinic strategically pursues its vision of establishing a medical home for the under- and

un-insured is yet to be finalized. As a constituent and supporter, if you have perspectives, ideas

The vision of the Genesis World Mission International Program is to lead, coordinate, and encourage

sustainable human health services through local and international partnerships in under-served

countries.

Genesis is about partnerships, sustainability and participating in solutions for the underserved. We

strive to empower organizations and communities – improving access to health care so that people

can provide for their families.

The Holistic WayOur approach in Kenya includes not just health care but a strong and ongoing emphasis on:

-education

-prevention

-partnering with the local community to find ways to deal with issues such as tainted water

sources and sanitation

-health solutions for the whole community going far beyond short-term medical teams

-partnering with other organizations, churches and faith based agencies to build networks

which will work with the local Kenyan community

-finding economic, feasible, and sustainable solutions for everything that we do

Ndubmeri: An Ongoing Model of Successful PartnershipOur long-term strategic focus began in Kenya. As part of a local coalition of churches,

individuals, and organizations, Genesis helped establish the PEFA Mercy Medical Clinic in

Ndumberi, which is a rural village just outside of Nairobi. The clinic opened in 2007 with a

high-quality, 6,800 square-foot medical facility.

The clinic has become very successful, very quickly and continues to serve up to 100 patients

daily, with Kenyan professionals providing medical and dental care. Our ongoing and steadily

decreasing financial support and working relationship with PEFA demonstrates our commitment

to developing a strong replicable, community driven model.

Why we Must Succeed (reported by Tracy Haworth)A Malindi child came into the procedure room for wound care. She had an extensive open and infected wound on her ankle. One of the

visiting team members, who is also a Genesis Board Member, asked me what would happen to her.

I had to answer honestly, even though I knew it was tragic.

In the US, she would have been hospitalized and would have received the “big guns” in the antibiotic world for at least a week. But not in

Malindi.

This child would most likely become sicker. Her wound needed more than oral antibiotics, but all we could do was to give her two days of

IV antibiotics and send her home on orals.

Given that she was undernourished, lived in a dirt hut, used dirty water from the local river, her chances of survival are minimal. This

infection will most likely go to her blood, possibly infect the bone, and she will lose her leg. And even more likely her life.

And what really makes me stop dead in my tracks and keeps me going in this work is this: when her wound was just a small scratch, a

consistent local health care presence could have treated it with a few days of a basic antibiotic, and then she could have been running

around, happy-go-lucky.

This is why we must press on towards helping this community have access to primary health care. Prevention and early treatment should be

available to the children in Malindi.

A New Project: Malindi, KenyaNow that the Ndumberi clinic project is entering its fourth

successful year of operation, we have begun to turn our sights

towards a new project. Thanks to a relationship with Eastwind

Community Church, we have been investigating how to

duplicate a scalable version of the Ndumberi effort on the

coast of Kenya, in the Malindi District.

Through two investigational trips in August and January, we

have strengthened our networks with several other

organizations, as well as the government of Kenya. Together,

we share common philosophies and offer core competencies

that together involve the local Kenyan community,

empowering them to bring health care solutions to their own

people.

Subsequent to our investigations, we scheduled a medical

team to go to Malindi in April. Due to the Icelandic volcano

eruption, only 2 of the 11 member team got to their

destination, with the others left at home in the US. However,

that random incident forced our hand to work with the local

Kenyan medical providers, businesses and several other

organizations to strengthen the network that has been in

development over the past 14 months. This model of

cooperation was put to a real test and its workability was

reinforced and we still managed to see 1000 patients.

Thankfully, in August, the Genesis team, working alongside

the six Kenyan providers who also returned to help, was able

to treat over 2,500 patients during a two week trip.

Sustainable PossibilitiesDuring the August 2010 trip, we made significant progress in

our development of a health care center in Malindi. The last

year of multiple meetings came to fruition as we signed a

Memorandum of Understanding with MAP International,

adding to the growing network examining how to help

Malindi’s health situation.

MAP International will now develop a Total Health Village in

the Malindi District, and Genesis is now a formal partner with

MAP and the local community to facilitate a health care center

where education and prevention will be provided. These

efforts will bring about a true community-driven model,

where the local villagers and local leaders will strategically

address human health issues in their community and arrive

with us on viable and sustainable solutions.
