INSTRUCTIONS:  This supplemental questionnaire is a required part of the application process for the above-listed examination.  Please review the job bulletin carefully and complete the supplemental questionnaire thoroughly for one of the specialty areas of desirable qualifications (ITIL Specialist, Telecomm Services Specialist, Business Security Specialist, or Shared Services Specialist) listed below. The information you provide will be utilized with the regular Job Application to evaluate your training and experience in the examination.

For each experience, please be sure to include the date range and duration of the experience, for example: “September 1999 through January 2000 (4 months)”. Failure to provide the date range indicating duration of experience will impact assessment of your qualification.

Please be sure you also define your specific level of responsibility (e.g., team member, system analyst, server/database administrator, team lead, or project manager; and for lead or project management responsibilities, include the number of persons reporting to you) and the platforms and various technologies used. Your responses to each question should be type-written or legibly printed and should be no longer than one page (8.5” x 11”) per question. ITIL Specialist:

Describe your ITIL Specialist experience instituting IT service management principals and frameworks such as ITIL, COBIT, and Six Sigma

Describe your experience working with project management and process management/integration methodologies including PMI’s Project Management Body Of Knowledge (PMBOK).

Telecomm Services Specialist:

Describe your experience with Carrier Services Telecommunications industry, with emphasis on the technical billing data for all voice/data services, and billing via tariffs and contract Universal Service Ordering Codes (USOCs).

Describe your experience in large on-line telephone billing systems, which include core elements of hierarchy structure allocation, compilation of multi-vendor billing codes generating a single product charge, and vendor billing account hierarchy functions.

Describe your experience managing and gathering detailed billing system business requirements to support ongoing software modifications.

Business Security Specialist:

Describe your experience in disaster recovery and business continuity, including the tools and techniques utilized.

Describe your experience leading DR-related projects and actual recoveries. Describe your hands-on experience in mainframe or open system discipline, including working knowledge of

storage management and networking.

Shared Services Specialist:

Describe your experience managing IT shared services implementation projects, such as email migrations, centralized file and print services, including developing and coordinating customer training requirements for hosted environment logins, email usage and unique application developments, such as SM Customer Relationship Database and MS SharePoint.

Describe your experience managing and maintaining customer account issues in the specific areas of change and service level agreement management on IT Shared Services products and IT asset management of desktop equipment.
