Adding a document

Document Template• Give your document a title• Choose your PDF• Click Add

• Click Edit to Edit the Template Info

• Any of these settings can be changed at any time.
• The document title will be referenced on every workflow.
• The file can be changed at any time. This makes insurance form changes very simple.
• The field font size and Page count are also available in the form builder.

• You have the availability to change the stamp anytime
• Team Notes: Use this to give your staff a note about how this form needs to be filled out.
• The note appears as a yellow sticky note on the workflow.
• Recipient Instructions: Use this to give information to clients, clinicians, and/or physicians about how the form needs to be filled out.
• This also appears as a yellow sticky note electronically.• Any note here will appear on the fax cover sheet when E-Faxed.
HCPCS Code: The HCPCS codes applied here will drive what is applied to the form on a workflowEx: L* is all L codes
Compliance Document: Check this box for a form that is collected only once per client (Ex: photo consent form, HIPPA..)
Insurance Relation: The insurance applied here will drive what is applied to the form on a workflow. (More information on next slide)
• NONE: Software automatically defaults to this | This will create 1 form with all Insurance that is pulled onto a workflow (as long as multiple fields are selected and applied to the form)
• ANY: This will create this form for each insurance listed
• SELECTED: This will apply specified Insurance to the field on the form

• There are multiple ways to open the field list: simply click Show Field List, click the black pointer arrow, or anywhere in the gray area

• At any time you can search for a field in the Search bar at the top
• Open your individual fields simply by clicking on the name
Signature Field
• The signature selections are for clinician, client, physician, and RTS• The field will populate a date when signed electronically underneath the signature
Custom Fields Text Field- Think as a sentence (Open to letters, numbers, symbols)Text Area- Think as a paragraph (Open top letter, numbers, symbols)Number- Defaults to no units, options to add unitsCheckbox- Creates a check |open to check one box or all of the boxesRadio Button- 2 options (circle or check)| Designed to force one choice to be checked or circled | Under master radio button, CLICK HERE to add your additional radio buttons (as many as you need, click)Drop Down Menu- Enter your custom choices in the field and separate by the pipe character (above the enter key)Date- Custom date | Option to change the date format | may use a default dateCircle Field- Produces a circleCurrent Date- Applies date that form is filled inCurrent Date & Time- Applies date & time that form is filled out

Diagnoses Field
• Diagnosos fields include all information that will automatically pull from the Brightree data and/ or data from MyForms onto the form
• Stacked Diagnosis codes.
Insurance Providers Field
• Insurance Provider fields include all information that will automatically pull from the Brightree data and/ or data from MyForms onto the form
• Stacked insurance policy IDs.
Evaluation Participants Field
• Evaluation Participant fields include all information that will automatically pull from the data in MyForms onto the form
Clinician Field
• Clinician fields include all information that will automatically pull from your Brightree data and/or MyForms data onto the form
Physician Field
• Physician fields include all information that will automatically pull from your Brightree data and/or MyForms data onto the form
RTS Field
• RTS fields include all information that will automatically pull from your MyForms data onto the form | The RTS/ USER assigned to the workflow
Items Field
• Item fields include all information that will automatically pull from your Brightree sales order or MyForms items onto the form
Workflow Field
• Date Needed• Actual Delivery Date• Sales Order # (External)• Date needed (pulls from MyForms)
Workflow Branch Field
• Workflow Branch fields include all information that will automatically pull from your Brightree data and/or MyForms data onto the form
Current User Field
• Current User fields: The active users information who is rendering the document (information will pull from MyForms)
Current Company Field
• Current Company fields: Company Name (If different from Branch Name)
• General Document fields: Apply a page count here
General Document Field
• Save & Close: Anytime during the form builder you can stop, save and close
Preview Template
• Selecting Preview Template at any time will show what the field will look like electronically and with your information applied
• Gear Box: This is where all of your options are to set this form (more detailed description on next slide)
• Copy: Applies all the settings you created in the gear box and copies
• Delete: Click this to remove the field

Change the font size from really small to really large (defaults to normal)
If you select “Yes” to the required option, the form will not be able to be sent out or returned when going electronically
Fill this out to apply a note to your staff about how the form should be filled out
This is produced when more items are allotted on the form. If checked, the value gets copied to corresponding field on the supplemental page
